**We are currently seeking applicants for a full-time temporary (6 month) Intensive Case Manager.**

**Role Overview**

Working under the direction of the Manager, Access and System Navigation Program, the Intensive Case Manager is responsible for assuming direct case management responsibility for higher acuity clients who have more complex needs. This will include developing a crisis plan advocating for the client, connecting the client with social and community resources, coordinating the client’s access to outside services and resources, potentially accompanying the client to appointments, and providing direct service to children, youth, and families. The Intensive Case Manager will also perform duties related to program development and evaluation, through developing effective linkages with service providers from larger systems.

This position is first dedicated to the transition of children, youth, and their families after the single point of access intake to community-based mental health services and supports. The position will work collaboratively with core service providers to support the clinical transition pathway. The position may evolve to include other transitional pathways from acute to community care. This position will provide service planning, navigation and brief supportive counselling to children and youth experiencing mental health concerns, and their families, to ensure they move in through and out of publicly funded mental health services in a timely and seamless manner.

**Qualifications**
- Bachelor of Social Work (Master of Social Work preferred) or equivalent in a mental health discipline and at least three years' related clinical experience or equivalent combination of education and intensive case management experience.
- Registered or eligible for registration in the appropriate professional college, if applicable.
- Excellent verbal and written communication skills to provide therapeutic services to clients and families.
- Excellent problem-solving skills to analyze complex client issues and recommend solutions.
- Demonstrated skills in therapeutic engagement.
- Good knowledge of MS Office Suites (Outlook, Word, Excel, PowerPoint).
- Ability to incorporate values of a culturally competent practice of diversity and inclusiveness in all aspects of work.
- Ability to work flexible hours, including evenings and weekends.
- This position is based at the Upper Paradise site. The ability to attend off-site appointments in client homes and other locations within the community is required.
- Certification in CHYMHs assessment is an asset.

Employment is conditional upon successful completion of background checks including police vulnerable sector check and proof of up-to-date immunization.
