**Position Summary**

The Inter-professional Primary Care Team (IPCT) will work collaboratively to provide primary care services to registered Primary Care clients and community clients, in various settings (at the CHC, on the CHC community health bus, at partner agencies or outdoors in the community) with a specific focus on marginalized populations. The IPCT team will provide services such as access to chronic disease prevention and management, mental health and addiction assessment, referral and counselling, as well as primary care clinics and outreach primary care services.

As a member of the Inter-professional Primary Care Team (IPCT), the Outreach Worker/Service Navigator supports the Outreach Team by providing support to connect vulnerable and marginalized clients with primary care and support services. This position will be part of a Nurse Practitioner led outreach team that will provide community based services. This position will also work with client referrals for service navigation to assist clients in connecting with non-medical services that supports their well being.

The Outreach Worker/Service Navigator will support clients in problem solving and will advocate for clients as appropriate. The Outreach Worker/Service Navigator will maintain knowledge of local resources and build strong, sustainable relationships with external agencies and partners. They will also maintain electronic client records according to organizational expectations and professional standards. The Outreach Worker/Service Navigator will be an essential member of the IPCT, who uses collaborative and innovative approaches within the broad determinants of health framework.

**Requirements**:

- Diploma in a health or social science discipline
- Experience in a health or social service setting.
- Experience working effectively with culturally, economically and socially diverse clients.
- Familiarity with social/ health services and systems in Brantford, Brant and Norfolk.
- Experience working with clients that experience mental health and addiction issues.
- Demonstrated ability to work independently and as an effective member of an interdisciplinary team.
- Strong organizational skills and communication skills
- Proficiency in the use of computers and various software programs (experience with Electronic Medical Records, EMRs, is an asset).

**Responsibilities & Skills**:

- Provide support to the IPCT and services to clients at a variety of locations (shelter and congregate settings, transitional and supportive housing, drop in locations, motels etc)
- Provide services through a relationship and community based framework
- Develop strategic relationships with various established and new community organizations that would be beneficial to IPCT clients.
- Work collaboratively with the IPCT and system partners (medical providers, shelters, hospitals, other community teams supporting similar clients) to develop, coordinate and execute care plans
- Demonstrated knowledge of mental health, concurrent disorders, treatment modalities, and harm reduction practices is an asset
- Strong communication (verbal and written), organization, and time management skills
- Demonstrated ability in conflict management and interpersonal relations
- Understanding of anti-oppression principles and ability to work effectively with people of various cultural, age and economic backgrounds
- Ability to work collaboratively within and across teams, developing and maintaining positive relationships with service providers
- Ability to work independently and in self-directed working conditions
- Able to work flexible hours primarily within a 5 day, Monday to Friday work week. Regular evening work 1-2 times per week.
- Includes centre based and offsite/outdoor work including visiting homes
- Able to travel within the Brantford-Brant-Norfolk OHT area
- Experience in working with homeless patients or those living with mental health and addictions is an asset
