Position Summary

Effective September 7th, 2021 all applicants must provide proof of full vaccination against Covid-19 (in the form of watermark receipts from the provincial portal) or provide written proof of a medical or religious reason for not being fully vaccinated as per the provincial direction as set out by the CMOH Directive 6.

Reporting to the Clinical Manager, Emergency Department, you will be responsible to ensuring that clients receive appropriate and timely access to and transitions across the continuum of care. In collaboration with patients and their families, internal and external partners, and within a Home First approach, the Navigator will work as part of an interprofessional team to ensure a coordinated, efficient and integrated approach to care delivery and transition back to the community.

**Responsibilities**:
Ensures safe and effective patient transitions (input, throughput and output) across the continuum of care
Promotes quality patient outcomes by facilitating effective processes of care and utilization of resources
Proactively identifies and addresses barriers to client transitions across the continuum of care
Leads engagement of patients, families, members of the interprofessional team, leadership and community partners in identifying strategies and action plans to remove barriers to transitions of care
Applies and models health literacy principles (i.e. Teach-Back)
Applies and models behaviour-based coaching methods
In collaboration with teams, effectively manages Bedflow Utilization tools to ensure timely patient transitions
Maintains an in-depth knowledge of internal and external resources and services to support client transitions to appropriate settings for care
Maintains an in-depth knowledge of Long Term Care (LTC), Community Care Access Centre (CCAC) and Alternate Level of Care (ALC) legislation and processes
Facilitate, lead and participate in appropriate client transition processes (action rounds, case conferences)
Enhance interdepartmental, inter-hospital and interdisciplinary collaboration, communication and problem solving
Develop primary and secondary specialization to support transitions with populations of interest and health systems

**Qualifications**:
Registered Nurse in good standing with the CNO
3-5 years clinical experience in a healthcare setting
Current experience in a Navigator role an asset
Experience using Bedflow Utilization Tools, PICIS, MediTech, MS Office, Excel
Effective interpersonal skills combined with the ability to work in collaboration with all members of the healthcare team, both internally and externally, across the continuum of care
Flexibility and adaptability to change is essential
Demonstrated ability to facilitate change and continuous improvement processes
Excellent organizational, planning and time management skills
