**Job Summary**:
The Relief Housing Support Worker will provide intensive case management under general direction of the Team Lead / Project Coordinator to the high acuity homeless population by utilizing using a trauma-informed care, strengths-based approach in all aspects of therapeutic community living. To provide quality of life and maintain the highest possible ability to function within the community.

**Responsibilities**:

- Ensuring the client is informed of Home for Good philosophy and Supportive Housing Policy Framework.
- Serve individuals in accordance with Coordinated Access and By-Name List policy and procedure, who are experiencing chronic homelessness with high acuity of need, and who may be prioritized in accordance with one or more of the following criteria: youth, indigenous population, and individuals transitioning from provincially funded institutions into homelessness.
- Work closely with Team Lead / Project Coordinator to facilitate access to available program supports, including mental health and addiction, nurse practitioner, life skills, coach, and community resources.
- Offer client engagement, intake, and identification of goals.
- Support clients to access and to maintain permanent supportive housing within the agency.
- Provide additional housing support, including neighborhood orientation, property management services, utility setup, assistance with rent payment.
- Support client to maintain tenancy and develop/maintain relationships with project staff and community resources to mitigate risk of housing loss.
- Support a "No Wrong Door" approach to service access by developing and maintaining partnerships with other community partners that contribute to an array of services related to secondary needs; making referrals as required.
- Broker services to support achievement of client goals and self-sufficiency.
- Provide strong advocacy for participants' self-determination and independence in day-to day activities.
- Provide a low-barrier approach; a service or housing which accepts people as they are, are easy to access, and are user-friendly as possible.
- Work closely with Regional Homelessness Team to ensure an effective system-wide approach, attendance at training and regular Regional Homelessness Team meetings.
- Utilize HIFIS (Homeless Individuals and Families Information System) reporting tool for accurate and timely records.
- Utilize OCANs (Ontario Common Assessment of Need) Assessment Tool.

**Requirements**:

- Bachelor degree, Social Service Worker or equivalent in social sciences.
- Three years of direct experience in the mental health field(s), case management, community development, homelessness, community support or supportive housing.
- Must provide proof of full vaccination against COVID-19.
- Must have a police clearance (with vulnerable sector screening).
- CPR Certificate/First Aid and Non-Violent Crisis Intervention Training.
- Knowledge of the mental health and addiction system.
- Knowledge of the Niagara Regions Homelessness Services
- Knowledge of the local community and the resources available.
- Group work, recreational programming and or outreach skills.   
- Sound knowledge of the local community and of services and supports available. 
- A valid driver’s license with permission to carry rider and reliable vehicle with 2-million-dollar liability.
- Strong written, assessment and record keeping.
- Demonstrate empathy and flexibility in responding to clients’ needs while fostering independence
- Politically and culturally sensitive.
- Strong moral and ethics, along with commitment to staff and client privacy.

**Work Conditions**:

- Working in a stressful environment where there are mental and emotional demands - this work is a high level of unpredictable work that could include life threating situations.
- Working with individuals who are experiencing poverty.
- Working with clients who may be experiencing mental illness and or addictions.
- May be exposed to but not limited to infectious waste, diseases, conditions, including exposure to the AIDS and hepatitis viruses.
- Interacts with residents, family members, staff, visitors, government agencies/personnel under all circumstances.
- Exposure to traumatic situations.
- Transporting clients in personal vehicle.
- Manual dexterity required to use desktop computer and peripherals.
- Intermittent physical activity including walking, standing, stairclimbing and sitting.
