RPN-Patient Care Coordinator

La Vie Medical requires a Patient Care Coordinator at each of our location, Ottawa and Kanata, to provide both direct and indirect patient care. The Patient Care Coordinator will perform technical and clinical support activities while assisting patients, and family members with concern and empathy. The Patient Care Coordinator will respect customer confidentiality and privacy. Patient Care Associate takes responsibility of referring telephone calls correctly ensuring timely communications.

The Patient Care Coordinator will be expected to establish and maintain open, effective communication between facility, clients / patients, physicians, treatment teams and nurses to ensure their effective completion of job duties.

**Experience**:

- Valid Registration and Liability Insurance as an RPN with Certification in Pharmacology.
- Current BCLS training.
- 2 years experience in the medical field.
- Effective communication skills with individuals at all levels of the organization.
- Able to deal with people sensitively, tactfully, diplomatically, and professionally at all times.
- Able to work efficiently as a part of a team as well as independently.
- Attention to detail in all areas of work.
- Good time management skills, multitasking skills, and the ability to prioritize tasks with mínimal supervision.
- Problem identification and problem resolution skills.
- Excellent internal and external customer service skills.
- Strong work ethic and positive team attitude.
