**Position Summary**:
The purpose of this position is to assist patients in achieving their highest level of functioning and independence, consistent with their values, priorities, capacities, and preference of care. The Discharge Planner will determine eligibility for services in accordance with applicable policies and procedures.

The Discharge planner will provide comprehensive clinical services to patients and families. This role involves crisis intervention, emotional support, consultation, counseling, completion of referrals and connecting patients to appropriate community services. The Discharge Planner takes a lead in working with patients and families, providing patient education, and counseling, and making referrals support services in an effort to address patients’ goals.

The Discharge Planner will support patients in the PREVENT program which is focused on primary prevention of cardiovascular disease. The PREVENT (Prevention Not Events) Clinic, powered by Manulife, is a multidisciplinary, risk factor reduction program for PRIMARY PREVENTION of cardiovascular disease, operated out of the current St. Mary’s General Hospital (SMGH) Cardiac Rehab Clinic at The Boardwalk in Waterloo, Ontario.

**Position Requirements**:

- Successful completion of a Master of Social Work Degree.
- Registration with Ontario College of Social Workers and Social Service Workers.
- Three (3) to five (5) years’ experience in the area of Social Work.
- Social Work assessment and exceptional counseling skills.
- Must be able to determine capacity in accordance with the Health Care Consent Act (1996)
- Knowledge of available community resources to support a safe discharge from hospital.
- Ability to work independently.
- Employee must demonstrate basic computer skills and literacy
- Excellent communication skills and ability to facilitate discussion towards conflict resolution.
- Commitment to maintaining and promoting healthy working relationships.
- Ability to work both independently and amicably in group situations
- Demonstrated skills in the areas of organization, communication, interpersonal relations, leadership, decision making and problem solving.
- Comprehensive appreciation and commitment to inherent values and ethics within the profession.

**Preferred Requirements:
- Acute care hospital experience (3-5 years).
- Knowledge of the community and resources available.
- Familiarity with electronic documentation systems.
- Excellent communication skills and ability to facilitate discussion towards conflict resolution.
