We currently have a **Regular Full Time **opportunity for a **Service Coordinator **in the **Outpatient Services** at our **St. John's Rehab Campus**.

**Hours of Work**:Weekdays : Days, 8 hrs; as per schedule

Summary of Duties
- Provide daily operational support for administrative staff in the coordination of referrals and bed management.
- Provide operational support for the program with a focus on implementing new processes, improving on established processes (e.g. referrals, intakes, scheduling), and improving communication.
- Liaise with referral sources to facilitate the efficient and timely provision of care and to maintain/improve program responsiveness to stakeholder requirements.
- Foster and maintain positive relationships with referring healthcare care facilities to ensure a quality continuum of care.
- Facilitate the identification, implementation, and monitoring of continuous improvement initiatives related to key operational processes.
- Facilitate, coordinate and monitor information sharing activities related to continuing education (grand rounds, teleconferencing, internal or external seminars, and discipline-specific courses).
- Participate in program development and assist in the development of new business opportunities related to Outpatient Services.
- Conduct and analyze referral data reports from different referral sources and contribute to ongoing

process improvement
- Other reasonable duties as assigned.
- Be aware of conditions and measures that may affect the safety of employees, patients, Sisters, students, volunteers, physicians and visitors including but not limited to: compliance with the safety standards of the Hospital and applicable legislation.
- Conduct their work in a safe manner; prevention, identification and reporting of safety issues and problems; taking corrective action, where possible; attendance at safety meetings / training / updates in accordance with the Hospital’s standards.
- May have access to confidential information related to Labour Relations.

**Qualifications/Skills**
- A Master’s Degree in Clinical/Health Sciences Bachelor’s degree required.
- Current certification of competency from a professional regulatory college
- Two years relevant experience within a health related setting.
- Excellent communication and organizational skills
- Ability to facilitate cooperative working relationships with multiple internal and external contacts
- Proficiency in the use of MS Office software products and other associated programs.
- Interest in research is an asset
- Experience with WSIB populations is an asset
