Rexdale Community Health Centre (RCHC) is a non-profit, multi-service, community-based organization that serves communities in Rexdale and North-West Etobicoke. Operating out of three locations, RCHC offers core services that include; primary health care, health promotion, community development programs, Pathways to Education and Diabetes Education Centre.
Founded in 2007, the Youth Association for Academics, Athletics and Character Education (Y.A.A.A.C.E) is a community organization that seeks to engage children and youth from all communities - particularly those that are marginalized and under-resourced through participation in year-round comprehensive programming and activities (academics, athletics, recreation, technology, and the arts). with a mission to use strength-based approaches to advocate for and create safer communities through programs and services that are accessible and culturally reflective and build capacity in all children and youth.
TO Wards Peace Project (TWP) is a risk driven, community-centric interruption model that takes a public health approach to interrupting and intervening in community violence. This model leverages the lived experience of affected communities to develop and implement effective community engagement approaches, innovative communication tools, social media strategies and focused investment geared at violence prevention and interruption. The integrated approach will combine community based, social media and hospital-based violence interventions. RCHC in partnership with YAAACE, the City of Toronto and other partners will oversee the development and implementation of the initiative in Jane Finch.
The model features intervention services led by **Violence Disruption Workers** who have lived experience and knowledge of working in racialized communities, which strengthens their capacity to build rapport and constructively intervene with youth most vulnerable to being involved in serious violence and crime. Staff will perform some of the work in an office environment but mainly in community, they will utilize critical thinking and problem-solving skills to mitigate safety risks and interrupt conflict within specified geographical areas. By engaging and developing trusting relationships with youth in their cohort, Violence Disruption Workers will support the building of their capacity/skills, setting life goals, connecting them to a relevant and culturally appropriate spectrum of services and shifting their views around community violence.

**Major Responsibilities**:
**Administrative**:
- Manage a caseload of 10 individuals and co design support and goal setting plans to address clients immediate needs
- Access a spectrum of culturally appropriate and relevant services
- Facilitate and support partnerships and collaborative work that benefit youth and their families.
- Work with diverse communities to develop resources for supporting well-being.
- Make appropriate referrals to TWP social workers and other appropriate community services
- Participate in mandatory training and personal capacity-building activities
- Required participation in therapeutic group sessions and regular one on one sessions to support personal wellbeing
- Collaborate with the TWP team to inform the development, design and delivery of the initiative
- Communicate and be responsive to Program Lead and other members of the initiative
- Documentation of engagements with youth and conflicts resolved within a determined note taking system.
- Engage with the evaluation team to inform the reporting and evaluation of the initiative
- Collaborate with online social media intervention models and hospital-based interventions where appropriate
- Work within the approved scope of work for the initiative
- Work collaboratively with intervention team, youth and their families to develop a customized care coordination plan
- Conduct intake and assessments in collaboration with Program Lead
**Outreach and Engagement**:
- Establish strong collaborative partnerships with all stakeholders and community partners
- Build and cultivate relationships with potential partners for supports and services that meet the needs of our clients
- Build awareness of the TO Wards Peace Initiative, along with the portfolio of services we provide through community education, including but not limited to facilitating presentations, workshops, and other public education and engagement events.
- Support and collaborate with Senior Management and Management to identify priorities and develop programs and partnerships.
- Participate and contribute to agency committees and initiatives that are related to the role
- Work collaboratively with school boards to support students through a pathway/journey which will positively affect their educational aspirations.
