Rexdale Community Health Centre (RCHC) is a non-profit, multi-service, community-based organization that serves communities in Rexdale and North-West Etobicoke. Operating out of three locations, RCHC offers core services that include; primary health care, health promotion, community development programs, Pathways to Education and Diabetes Education Centre.

TO Wards Peace Project (TWP) is a made-for-Toronto risk driven initiative focused on addressing elevated level of gun violence in Northwest Toronto. The TWP Project Lead will oversee and support the implementation of the project in the Rexdale area. This will include contributing to program design and development, support to interventions, research and documentation, representation and partnership development, and training, mentoring and organizational learning for implementing Youth Peace Projects.

We are looking for an individual with project/program development, implementation and supervision experience to support this initiative.

**Responsibilities**:
**Program Administration**:

- Oversee the delivery of all aspects of the program including the execution of the Peace Projects, Public Communications Strategy, Compassionate Fund, and Risk Mitigation Framework.
- Develop and update program materials
- Lead and facilitate Coordination Meetings and other TWP-related meetings - ensure all TWP partners engage and provide / share updates, challenges, and successes in the development and delivery of the program.
- Support, administer and manage the disbursement of all program funds
- Oversee the tracking and reporting of all key program deliverables.
- Supervise and lead hiring process for TPW staff including:

- Ensure that all roles/responsibilities are clearly outlined for all roles within TWP.
- Collaborate with City and other TWP partners to build relationships with institutional partners (TPS, TCHC, TDSB, TCDSB, RMYC and others etc.)
- Explore and determine BRAVE and FOCUS referral pathways to the TWP model

**Reporting, Work plans & Budgets**
- Report and document all changes to the work plan and/or budget.
- Report on all TWP expenses
- Support Grassroots to ensure quarterly reports, work plans and budgets are completed by the deadline
- Support Grassroots in achieving their deliverables and reporting adequately.

**Grassroots Engagement**
- Provide mentorship and oversight for both Grassroots agencies Rexdale and Jane and Finch) through a trustee agreement and/or documentation processes
- Build and maintain rapport with Grassroots staff
- Support Grassroots with conflict mediation should challenges between parties arise.
- Support in the development of supervision and support plans for key staff and VDWs.
- Ensure Social Worker collaborates with Grassroots to inform relevant supports required for cohort.
- Support with outreach and connections with community agencies, facilitate presentations to develop and maintain partnerships for referral opportunities.
- Support in informing the case management system, outreach plan and referral processes/pathways for cohort participants.
- Support Grassroots to ensure data is collected, recorded and is secure and protected
- Ongoing check-ins with Grassroots to highlight supports required for cohort members - leveraging Social Workers as part of the support system.
- Ensure culturally relevant and holistic approaches are embedded in practices.
- Provide management and budgetary oversight of the fund with the City of Toronto and Grassroots for Budgets and Compassionate Fund
- Monitor and revise as needed eligibly, approval processes and guidelines for Compassionate Fund
- Maintain a tracking system for expenditure and participant use of compassionate funds.
- Support Grassroots Program Leads in monitoring, recording and distributing funds
- Conduct ongoing check-ins with Grassroots to identify training opportunities - communicate and provide updates on potential capacity-building supports
- Leverage relationships and partnerships to support the development and implementation of the training and capacity building plan.
- Identify capacity building opportunities - share updates, recommendations with TWP partners.
- Work collaboratively with a Training consultant to deliver Community of Learning Practice activities - promote and build awareness on public health approaches to mitigate violence.

**Peace Projects**
- Provide trusteeship and mentorship support to Peace Project groups
- Support groups in tracking and reporting on their program deliverables, budgets, successes, and challenges.
- Leverage existing relationships and partnerships to support the implementation of Peace Projects

**Monitoring and Evaluation**
- Work with external evaluator and the City of Toronto to support in the development of a monitoring and evaluation framework to inform successful program development and implementation.
