Etobicoke, Canada Rexdale Community Health Centre Full time

This role specifically supports gun/gang involved, impacted, or affiliated young people between the ages of 12-29 who are disengaged from education or incarcerated in a youth justice detention facility, provincial or federal institution and reintegrating back into community

This role will:

- Build, create and manage partnerships and connections with systems/institutions (TDSB/TCDSB, youth justice detention facilities, and Provincial and Federal correctional institutions etc. that provide supports for youth who are gun/gang involved, impacted, or affiliated
- Facilitate group outreach and engagement workshops and programming geared to gang/gun involved, impacted or affiliated youth who are incarcerated or connected to TDSB/TCDSB
- Provide supports for young individuals facing disengagement from education, training, employment and society
- Support in the development of a comprehensive partnership strategy to establish collaborative relationships that align with our organization's mission and goals
- Provide and coordinate essential services (reintegration, education, housing, mental health and well-being, risk mitigation, violence prevention programming, culturally responsive and reflective services) youth in justice detention facilities, adult detention facilities, transitioning back into community and TDSB/TCDSB youth in communities identified as being engaged by the criminal justice system (from Rexdale/Jane and Finch)
- Work collaboratively with partners to design, develop, and implement joint programs and initiatives that address the needs of disengaged/incarcerated youth.
- Initiate, build, support and foster formalized partnerships with City of Toronto, Provincial and Federal Correctional institutions, Toronto District School Board and Toronto Catholic District School Board,.
- Initiate, build, and foster relationships with other institutions that work with incarcerated and disengaged youth.
- Create, provide and facilitate gun/gang prevention, intervention & interruption workshops/activities and case management as needed)
- Work within a through-care model to provide continuity of care for incarcerated youth and youth re-entering community post release by providing a triaging period for youth after release and before re-entering the school environment and transitioning back into community.
- Work collaboratively with the TDSB/TCDSB and TWP project partners to identify Black youth disengaged from education and training (NEET) and coordinate with various TWP project partners to engage this vulnerable cohort of youth to education, training, mentorship, reflective services/cultural consideration of care services etc.
- Collaborate with Probation Officers, Parole Officers, Social Workers, community agencies and School Admin to develop successful rehabilitation and reintegration plans
- Collaborate with Caring and Safe Schools to assess research, evaluate, implement, and operationalize various violence prevention and intervention initiatives relating to TDSB/TCDSB students
- Support and assist Caring and Safe Schools (TDSB/TCDSB) in matters pertaining to school-based violence prevention and intervention strategies
- Advocate for the needs of disengaged youth and promote collaboration with key stakeholders to address the challenges they face
- Coordinate workshops, group sessions, training sessions, and information-sharing events to promote knowledge, build capacity and empower youth in institutions
- Maintain accurate records of partnership activities, communications, and outcomes
- Generate regular reports to demonstrate the impact of partnerships on violence prevention and interruption
- Provide culturally appropriate prevention, intervention, and interruption services to gun/gang involved, impacted or affiliated
- involved incarcerated young people connected to Rexdale and Jane & Finch.
- Offer pre-release support, planning, and coordination to clients in youth justice detention facilities, provincial jails, and/or federal institutions
- Connect incarcerated individuals to TWP supports (VDWs), therapists, and additional reintegration support upon release
- Provide one-to-one case management for critical/emergency situations
- Deliver trauma-informed care and culturally responsive services and mentoring to incarcerated youth
- Facilitate restorative justice practices with clients
- Support clients in creating stability for themselves and their families through appropriate referrals for counselling, financial counselling, job readiness programs, and post-secondary education
- Provide access to the Community Safety staff (VDW and TWP stakeholders) to support principals, administrators, liaisons, educators, etc. mitigate issues of violence in schools
