ICAN is a community based non-profit organization providing services and resources to persons living with permanent physical disabilities, persons living post-stroke and older adults. ICAN provides assistance to live independently and inclusively in the community through individualized support and rehabilitation services.

JOB SUMMARY:
The Client Services Coordinator (CSC) for Supportive Housing is responsible for overseeing the delivery of high-quality, client-centered care within ICAN’s supportive housing program. This role involves coordinating care plans, finding resources and other community support services to ensure our clients receive the support required to live independently and inclusively in the community. The CSC is a point of contact, resource and support system for not only the clients but our independent living assistants. The CSC is responsible for fostering a collaborative and empowering environment for both staff and clients.

KEY RESPONSIBILITIES

Care Coordination:

- Develop and manage individualized care plans for clients in the supportive housing program, ensuring that services meet their physical, emotional and social needs.
- Ensure the delivery of client-centered services that respect the dignity and choices of each client.
- Conduct regular client reviews and collaborate with system care coordinators, health professionals, and other stakeholders to ensure services are meeting the needs of clients.
- Serve as a liaison between clients, families, and external partners, advocating for client needs.
- Ensure the quality and effectiveness of services by conducting regular assessments and modifying care plans as needed.
- Regularly visit clients to assess their satisfaction, address concerns, and gather feedback to improve service delivery.
- Provide direct support and training to clients as needed.
- Maintain updated and accurate client records and ensure confidentiality.

Collaboration:

- Work alongside a team of Independent Living Assistants (ILA’s) providing guidance, support, and education to ensure optimal service delivery.
- Work collaboratively with the scheduling department to ensure adequate coverage for all supportive housing units.
- Foster a positive work environment that encourages teamwork and professional growth.
- Work closely with healthcare providers, community organizations, and internal ICAN departments to ensure seamless service integration for clients.

Compliance and Reporting
- Ensure that all services meet the standards set forth by ICAN, CARF and ministry.
- Ensure compliance with all relevant regulations and standards.
- Stay informed about changes in regulations and best practices.

QUALIFICATIONS
- Certification, Diploma or Degree in health care, social work, personal support services, or other related field.
- Minimum of 2 years experience in healthcare, social services, community support services or supportive housing.
- Strong knowledge of disability services, rehabilitation, or related fields.
- Excellent problem solving, organizational, and communication skills.
- Ability to work collaboratively with staff, clients, and community partners.
- Proficiency in using relevant software and technology.

Bilingual (French & English) considered an asset.

WORKING CONDITIONS
- Full-time permanent position, typically Monday to Friday, with occasional evening or weekend work as needed.
- Work is primarily performed in an office and in supportive housing units.
- May involve some travel for community outreach and professional development.

PHYSICAL REQUIREMENTS
- Ability to perform physical tasks related to client training and support.
- May require lifting or moving equipment and assisting clients with mobility.

Compensation: In accordance with the terms outlined in the OPSEU collective agreement
