**Position Description**:
**The Occupational Health & Safety Department has an immediate need for a Program Manager.**:
**Here's What You'll Do**:

- Review and follow up on incident reports related to workplace violence _
- Facilitate follow up including root cause analysis (RCA) or other event investigations on workplace violence - related events. Partner with local leaders to assist and coach leaders on follow up process.
- Develop a streamlined reporting process to accommodate all our current workplace violence reporting processes (e.g. employee safety event, code white event, security event, patient event).
- Review applicable Security reports to ensure workplace violence incidents are being captured accurately and work with Protection Services to link their security reports to safety reports (employee events and/or Code white reports).
- Review and follow up on safety reports related to workplace violence prevention - ensure appropriate classification of events and corrective action is taken. Assist and coach local leaders in responding and following up on events to prevent recurrences.
- Respond to Code White incidents during regular business hours to identify gaps/opportunities for improvement or identify leading practices. Follows up with unit to ensure debriefs and safety reports are completed.
- Participates in the Code White review Committee.
- Employee Relations will conduct workplace investigations for staff-to-staff aggression/violence incidents. For staff aggression/violence to patients/caregivers, Employee Relations will be engaged to investigate the matter in collaboration with OPFE.
- Data Monitoring_
- Review workplace violence metrics and select key indicators to track related to workplace violence prevention.
- Prepare and analyze data and reports related to the effectiveness of workplace violence prevention programs and initiatives.
- Workplace Violence Prevention Program _
- Coordinate workplace violence efforts across the organization so there is greater alignment - bringing together the leads of the many groups addressing aspects of workplace violence including:

- Staff to staff aggression/violence.
- Patient to staff aggression/violence.
- Family caregiver to staff aggression/violence.
- Public to staff aggression/violence.
- Lead the violence QIP action plan with input from key stakeholders.
- Participate with quarterly case review of flagged charts of patients to determine whether risk flags should be removed.
- Lead the Required Organizational Practice on Prevention of Workplace Violence for Accreditation with input from key stakeholders.
- Network with community partners regarding workplace violence prevention; to discuss common concerns in relation to the issue of workplace violence prevention.
- Committee Support _
- Support the Employee Relations team with staff-to-staff violence prevention by:

- Triaging staff to staff aggression/violence incidents to Employee Relations.
- Collaborating with Employee Relations on incidents of staff aggression/violence to patient/family caregiver.
- Partnering with Employee Relations on the collection and analysis of violence data.
- Providing updates/changes to processes and procedures.
- Assisting with policy and procedure consistency.
- Supporting/partnering on staff education.
- Support the Patient Escalation Oversight Committee by:

- Conducting chart reviews related to incident investigations.
- Conduct audits of the Behaviour Escalation Screening Tool (BEST).
- Working with units to enhance compliance with completing BEST tool and associated care plan.
- Provide support to units for patients with risk of behavioural escalations.
- Support the Family Caregiver Escalation Steering Committee by:

- Helping managers to assess situations (level of behaviour) and
- Coach managers on how to have difficult conversations with Family Caregivers when needed to build capacity, including in some cases partnering with them to have the discussions with the families.
