As a **Clinical Care Partner**, your typical day will impact the following ways:
- In collaboration with the Regional VP, Executive Director and DOC, provide follow-up and support to all care community team members in order to ensure that care meets and/or exceeds the organization’s and provincial health authority requirements.
- Assist in the development of compliance plans as required, and follow up on compliance actions plans in consultation with the care community team members.
- Support the development of processes and evidenced based practicesLead care community and organization-wide improvement processes/projects as directed and in consultation with the Regional VP Operations and care community team members.
- Assists and coach care community team members at all levels to improve processes, strategies and activities in relation to care and services through the provision of education and orientation.
- Provides recommendations regarding training and education, which would be of benefit the care communities and the organization as a whole
- Work in accordance with the organization’s Health and Safety Policies and Procedures and in compliance with the Occupational Health and Safety Act;
**Education and Experience Required**:
- Must be a Registered as a Registered Nurse (RN) in good standing with the College of Nurses of Ontario
- Post Baccalaureate education in Nursing, Health Sciences or related field is preferred
- Certificate of Gerontological Nursing (Canadian Nurses Assoc.) preferred
- Knowledge and understanding of provincial regulatory requirements and inspection process
- 3 years related management and relevant experience in long term care
- Evidence of ongoing professional development and a demonstrated commitment to continuous learning
- Highly motivated; demonstrates an ability to be self-directed, working independently and as a member of a collaborative team
- **_Able to drive and/or travel to long term care communities - access to a reliable vehicle and valid G licence is required_**
