***Role Overview**
The Canadian Mental Health Association - Halton Region Branch (CMHA-HRB) is seeking a full-time Case Manager, Dual Diagnosis (DD) to provide service in Halton, Ont., which is comprised of Oakville, Burlington, Milton and Halton Hills. The position is a 1-year contract position at 37.5 hours per week within an integrated team of individuals who work closely together to provide meaningful programs to people living with mental health issues who have a coexisting developmental disability (Dual Diagnosis).
The Dual Diagnosis program strives to support people with mental health concerns and developmental disability to:
- Live full and meaningful lives in the community
- Enhance coping strategies, life skills and social skills
- Create a strong network of reliable and supportive relationships
- Have choices about where to live and work
- Receive services that are accountable to the person and family supported and are available as needed
- Provide individual 1:1 support coordination to clients with mental health concerns and developmental disabilities; concerns related to substance use may also be present
- Focus on strategies to integrate clients into the community
- Make referrals on behalf of clients to a wide variety of community programs and services
- Support individuals within the mental health and developmental system by empowering them, identifying goals and strengths and successfully implementing formal and informal supports
- Promote the development of enhanced capacities
- Utilize a holistic approach to recovery to support client’s enhanced functioning, community involvement and establishing healthier relationships
- Commit to supporting health, safety and wellness according to relevant legislation and agency policies and procedures
**Responsibilities**:
- Responsible for all aspects of case management and maintenance of client records
- Develop and work in a service for those with a significant mental health issues and a developmental disability
- Work with individuals to create a holistic service plan, focusing on their strengths and capacities
- Engage in proactive planning with individuals with dual diagnosis, including developing individual treatment and service plans that address the individual’s specific needs. This may involve crisis plans, support and diversion plans with criminal justice system as well as community support plans
- Interact and support families where appropriate and needed
- Work collaboratively with community partners in a multidisciplinary approach
- Provide mental health and developmental disability education and advocacy
- Other duties as assigned
**Qualifications**:
- University degree in social work, psychology, occupational therapy or a relevant field
- Certificate in Dual Diagnosis would be an asset
- Regulated health professional (registered social worker or equivalent) an asset
- At least three (3) years’ experience working directly with individuals with complex mental health concerns and developmental disabilities
- Commitment to the Recovery philosophy of service provision
- Experience providing community outreach and must possess knowledge of community resources and services
- Able to work effectively independently and/or within a team environment
- Excellent oral and written communication skills
- Demonstrates strong initiative, creativity, flexibility and an openness to change
- Proficiency in MS Word and Excel
