Description

This is a casual relief position with no guaranteed hours. Flexibility in scheduling is required. The work schedule is determined in consultation with your supervisor and may vary according to requirements of responsibility. Applicable shifts include a ½ hour paid meal break.

KEY RESPONSIBILITIES:

ACCOUNTABILITIES:

Develop and implement case management of assigned clients/residents; assist clients/residents in establishing goals, monitoring progress, and preparing discharge plans. Provide back-up support to front line positions and their functions as required. Respond to day to day “urgent” needs of clients. May be required to attend resident appointments. Attend and facilitate case conferences with case management team. Collect and record all case management statistics on a daily basis. Prepare monthly statistical report for Program Services Manager. Prepare monthly outcome measures report for Program Services Manager. Review and forward weekly resident update reports to the Region of Halton. In consultation with Program Services Manager review all resident intakes, discharges and restrictions. Conduct tours of building for groups or individuals as requested.

Perform other duties as required.

WORKING CONDITIONS:

May work with potentially aggressive clients.

QUALIFICATIONS AND EDUCATION REQUIREMENTS:

EDUCATION, QUALIFICATIONS AND CERTIFICATIONS:

Post-Secondary education such as Social Service Worker Diploma. Possess current WHMIS, Non-Violent Crisis Intervention, First Aid and CPR certifications. Valid Ontario Driver’s License. Alternative combinations of education and experience may be considered.

EXPERIENCE AND KNOWLEDGE:

One year of previous experience working in a social services residential setting. Competent on computer equipment such as Windows, Microsoft Office, HIFIS etc. Demonstrates knowledge of Community-based resources.

SKILLS AND CAPABILITIES:

Excellent oral and written communications skills and effective interpersonal skills. Ability to establish and implement goals and plans for self and clients. Maturity and ability to exercise sound judgment, strong leadership skills and effective intervention skills. Ability to demonstrate a supportive, respectful, and non-judgmental approach to clients. Ability to develop and maintain a team environment. Demonstrated ability to be self-motivated and to work both independently and under supervision. Ability to lift/move up to forty (40) pounds. Ability to undergo applicable screening and background checks successfully that are satisfactory to The Salvation Army, in its sole discretion (i.e., The Salvation Army Abuse Registry) and provide an original copy of a Background Check with vulnerable sector Ability and willingness to develop an understanding and support for the mission and purpose of The Salvation Army in Canada and its implications as related to position responsibilities.
