Reporting to the Manager, Student Rights & Responsibilities (SRRO), the Case Management Specialist (CMS) leads a multi-disciplinary Care Team (10+ members) through a structured needs assessment and coordinated intervention planning process for students with acute and/or complex needs. This includes supporting students with issues involving hospitalizations, prolonged absences, physical and mental health challenges, experiences of trauma, lack of basic needs including income, housing and food insecurity, and access barriers to needed services. The CMS provides case management and risk assessment supports for students in collaboration with other members of the Care Team. Through the early identification of student concerns, the CMS establishes a baseline for risk, provides holistic support and coordinated care by consulting with internal and external partners.

The CMS is responsible for case documentation, recommending interventions and the development of safety and wellness plans. The CMS meets directly with students to monitor and assess progress and determine any required adjustments to individual care and wellness plans. Utilizing evidence-based best practices, the CMS also supports the development and implementation of capacity building programs, communication initiatives, and training for Sheridan faculty and staff to recognize indicators of a student in need of support and provides guidance and support in student crisis or critical incident situations. The CMS works within the broader Student Affairs department and coordinates services for students requiring specialized intervention and coordination of support for areas including but not limited to: Wellness and Counselling, Accessible Learning, International Services and acts as a point of referral for Residence and faculty around students facing challenges with safe and constructive engagement in the College’s learning and living environments.

**What You’ll Be Doing**
- Providing case management support to students with acute, complex needs who may face barriers to service access. This includes the provision of intake, triage, assessment, coordinated care and support through consultation with internal and external partners, developing individual support plans, monitoring, and following up on progress and transition planning;
- Establishing a baseline for risk utilizing structured assessment tools and clinical/professional judgement, and response to signs of psychological distress, including of suicidality and potential risk of harm to others;
- Developing and coordinating all operational components for Care Team meetings, presenting case updates (needs assessment, intervention strategies, risk assessments, progress evaluations) as well as coordinating all care plans and interventions;
- Providing training, advice, guidance, and support regarding triage, active student cases, consultations and interventions;
- Supporting the Case Management Facilitator with their participation on CARE team, documentation in a shared case management database and supporting with the escalation process for elevated/critical risks/situations;
- Maintaining confidential case notes documenting all actions, conversations, interventions, implementation plans, progress reports, and following up within the case management database while ensuring proper record-keeping and file retention is in accordance with best-practice standards and based on FIPPA and PHIPPA guidelines;
- Analyzing data and providing statistical trends analysis to generate end-of-year reports that illustrate trends related to student support;
- Participating and engaging with institutional campus-wide internal committees/working groups (i.e., Sexual Violence Taskforce and Sheridan’s Threat Assessment Team) as well as external community agencies (i.e. Peel Situational Table, and Halton Transitional Youth Committee) to support students’ wellbeing;
- Developing awareness, education, communication strategies and marketing/educational material in collaboration with campus wide stakeholders and delivers training and education sessions on student health, mental health and success;
- Participating in other projects and activities as assigned including support for new student orientation and Student Affairs initiatives as well as other special projects and research assigned by the Manager, SRRO.
