The Toronto Grace Health Centre (TGHC) provides, specialized care to those individuals who require Complex Continuing Care, Post-Acute Care Rehabilitation, Transitional care, and Palliative Care services. The care and services provided at TGHC help facilitate the patient flow through the health care system and ensures recovery to those individuals who may return to their homes or to Long Term Care (LTC) facilities in their communities. TGHC operates in 3 sites - the Main site (Church & Bloor St) and our Harbour Lights site and Specialized Care Centre (Victoria Park & McNicoll). **AFTER HOURS CLINICAL SUPERVISOR - HARBOUR LIGHTS**

**Full Time & Part Time -1-year contract**

**Position Summary**:
The Patient Care Department at our Harbour Lights, Scarborough location is currently seeking an After Hours Clinical Supervisor. The After Hours Clinical Supervisor is an active member of the interdisciplinary team and is responsible and accountable to provide supervision and leadership to the patient care units. The After Hours Clinical Supervisor has clinical and leadership expertise in the provision of quality care for diverse patient populations including residents living with complex mental health illness & elderly patients with frailty and dementia in a transitional care setting. They will collaborate and support the inter-disciplinary team with the after hours daily clinical and operational functions of the units.

**DUTIES AND RESPONSIBILITIES**:

- Assigns, supervises, and guides staff in providing high quality care and services including accurate clinical assessments, treatment plans, documentation, and participation in patient case conference meetings.
- Adheres to the collective agreements at Toronto Grace Health Centre
- Assists clinical staff on units as needed to demonstrate/teach new skills or new equipment.
- Assist Patient Care Manager in the recruitment, orientation and retention of nursing staff.
- Champions quality improvement initiatives including patient and family centred care, safety huddles, BPSO initiatives and falls prevention.
- Promotes a safe and efficient working environment by adhering to organizational policies, guidelines, infection control and standard precautions.
- Maintains confidentiality and builds a respectful working environment.
- Other duties as required.

**QUALIFICATIONS**:

- Bachelor’s Degree in Nursing; certification in Gerontology or Rehabilitation Nursing an asset.
- Certificate of Competence issued by the College of Nurses of Ontario; entitled to practice with no restrictions.
- Current Basic Cardiac Life Support (BCLS) Certification
- Minimum of 3 to 5 years working with persons with Dementia or cognitive impairment.
- Minimum of 3 to 5 years working with persons with Mental health experience, hospital-setting and with diverse communities preferred.
- Proficient in the use of an Electronic Medical Record
- Strong, comprehensive knowledge of evidence based clinical practice.
- Self-directed; ability to handle a wide variety of clinical and administrative functions while maintaining a high degree of professionalism, maturity, discretion and judgment.
- Strong interpersonal and communication skills
- Knowledge of the RAI-MDS
- Demonstrated proficiency in Microsoft Office, Word, PowerPoint, Outlook, Excel
- Excellent attendance record
