**1-Temporary Part Time (12-month contract) (Weekends and Holidays: 11 pm to 7 am)**

The Toronto Grace Health Centre (TGHC) provides, specialized care to those individuals who require Complex Continuing Care, Post-Acute Care Rehabilitation, Transitional care, and Palliative Care services. The care and services provided at TGHC help facilitate the patient flow through the health care system and ensures recovery to those individuals who may return to their homes or to Long Term Care (LTC) facilities in their communities. TGHC operates in 3 sites - the Main site (Church & Bloor St) and our Harbour Lights site and Specialized Care Centre (Victoria Park & McNicoll). **AFTER HOURS CLINICAL SUPERVISOR - TRANSITIONAL CARE PROGRAM (ONSITE)**

**Full Time & Part Time: 1-year contract**

**Position Summary**:
Our Transitional Care Program located in Scarborough, is currently seeking an After-Hours Clinical Supervisor. You will be an integral part of a collaborative, inter-disciplinary team focused on delivering seamless services and programs to our clients while providing after-hours clinical and operational support. The After-Hours Clinical Supervisor has clinical and leadership expertise in the provision of quality care for diverse patient populations including clients living with mental health illness, the frail elderly within a transitional care setting. They will collaborate and support the inter-disciplinary team with the after-hours clinical and operational functions of the units. This is a great opportunity to demonstrate your leadership skills in the delivery of efficient and effective client care.

**DUTIES AND RESPONSIBILITIES**:

- Be the first point of contact for staff afterhours and on weekends to report clinical concerns and escalate to administrator on call as appropriate.
- In conjunction with hospital staff and in accordance with hospital policies, protocols, and procedures, manage client and family complaints and concerns.
- Maintain communication with building security and engineering regarding relevant concerns.
- Assist EMS as it relates to transferring clients to the ER
- Adheres to the collective agreements at Toronto Grace Health Centre
- Assists clinical staff on units as needed to demonstrate/teach new skills or new equipment.
- Supports quality improvement initiatives including patient and family centred care, safety huddles, BPSO initiatives and falls prevention.
- Promotes a safe and efficient working environment by adhering to organizational policies, guidelines and infection prevention and control practices.
- Is familiar with emergency and evacuation procedures.
- Maintains confidentiality and builds a respectful working environment.
- Other duties as required.

**QUALIFICATIONS**:

- Bachelor’s degree in nursing; certification in Gerontology or Rehabilitation Nursing an asset.
- Certificate of Competence issued by the College of Nurses of Ontario; In good standing and entitled to practice with no restrictions.
- Current Basic Cardiac Life Support (BCLS) Certification
- Minimum of 3 to 5 years working with persons with Dementia or cognitive impairment.
- Minimum of 3 to 5 years current experience working with persons with Mental health conditions.
- Proficient in the use of an Electronic Medical Record
- Strong, comprehensive knowledge of evidence based clinical practice.
- Self-directed; ability to handle a wide variety of clinical and administrative functions while maintaining a high degree of professionalism, maturity, discretion and judgment.
- Strong interpersonal and communication skills
- Knowledge of the RAI-MDS
- Demonstrated proficiency in Microsoft Office, Word, PowerPoint, Outlook, Excel
- Excellent attendance record
