**Salary Range is $52,753.45 to $63,158.47 per annum**

**We are currently seeking a **full-time Community Support Worker, for a 6 month contrac**t, to work as part of a Housing First Project called “Home for Good in Peel”.** This project is a collaboration between Services and Housing in the Province, Ontario Works, John Howard Society of Peel Dufferin, Salvation Army in Peel and CMHA. This role with operate in tandem with the Peel Outreach program, which is part of a continuum of services that seek to end homelessness in Peel Region and increase access to mainstream services. Clients are individuals and/or families who are homeless or are at imminent risk of homelessness and experience serious mental health challenges.

The position is a full-time **6 month contract position of 37.5 hours** per week and will be based out of the 60 West Drive, Brampton, Ontario.

CMHA Peel Dufferin has adapted a hybrid working model including a mix of on-site and remote work. All employees are required to have onsite presence 2 to 3 times per week, or more if needed as per program needs.

**What You Will Do**
- Connect individuals with housing, including accompanying them to view potential housing options and being present on move-in day.
- Facilitate start up shopping for basic groceries and household supplies upon move in
- Ongoing intensive case management with participants to set goals and create plans to reach those goals.
- Complete standard assessments and re-assessments at the appropriate date(s) and other documentation as required.
- Maintain professional relationships with referral sources and other service providers.
- Minimum of weekly home visits with housed participants to offer support, connect to resources, etc.
- Advocacy with participants as required (i.e. Landlord relations, EIA, Residential Tenancy Board,
- Life skills teaching to help build Client capacity to maintain housing, especially focusing on tenancy skills and knowledge.
- Develop and maintain relationships with landlords to increase participants housing success and contribute to the creation of the Outreach Landlord List.
- Maintain accurate records and statistics, entering data into information systems as appropriate.
- Record hours and maintain detailed record of activities and files for program participants.
- Report monthly statistics for pilot project.
- Support the after-hours program functions such as the Outreach Van Shift and Street helpline.

**What You Will Need to Join Our Team**
- Post-Secondary diploma or degree in Social Services or degree in relevant discipline (BSW preferred)
- Minimum 3 years of experience with the homeless in an outreach or shelter setting
- Minimum 3 years of Case Management experience.
- Strong knowledge and demonstrated understanding of mental illness
- Demonstrated skills in performing assessments and on the spot risk assessments
- Comprehensive knowledge of community mental health and human service networks in the Peel region
- Non-violent crisis intervention, ASIST, CPR and First Aid Certification
- Satisfactory police records search and vulnerable sector screening
- Satisfactory references
- Valid driver’s license and access to a reliable vehicle
- Ability to work regular evening shifts, and some weekend shifts as needed.
