**Role Overview**

**About the Team**

The Lawrence East Partnership program is a comprehensive and collaborative support service for residents who are vulnerable and have complex needs living at two Toronto Community Housing buildings, 4175 and 4205 Lawrence Avenue East in Scarborough.

**About the role**

Using a hub model of service, this position will be a part of a multi-disciplinary team who will work together to ensure that skill enhancement and services are in place for residents to maintain their housing and to improve their physical, mental health, and substance use challenges.

The Case Manager - Mental Health will seek input and work in partnership with the client to development comprehensive assessments and treatment plans. Provide client
- centred case management supports to residents living in 4175 and 4205 Lawrence Avenue East and also assume responsibilities for coordinating and/or facilitating client group sessions. This case manager will have specific expertise enabling them to provide education, information and support to promote hope and the pursuit of personal recovery.

**Summary of Duties**:

- Conduct face-to-face assessments with clients at their locations including but not limited to home, shelters and hospitals
- Interact directly with referral source, physicians, and other services involved in client's care
- Initiate services in partnership with the client/s at the point of intake to ensure that proper assessment and initial service response occurs
- Develop and lead coordinated care plans with input from the client/ is and complete the Ontario Common Assessment of Need (OCAN) and other assessments as required
- Receive input from the client/s to facilitate a discharge when appropriate by developing a comprehensive discharge plan
- Collaborate with other systems, organizations and individuals from courts, probation, safe beds, hospital and community to ensure a well-coordinated service system
- Record client status/progress using the appropriate documentation system

**Summary of Qualifications**:

- A combination of a post-secondary degree or diploma with 4 years direct experience in the delivery of community mental health services
- Sound theoretical and practical knowledge of mental illnesses
- Sound clinical knowledge and understanding of the client population and demonstrated ability to work independently and effectively with individuals with serious mental illness and complex needs
- Direct experience in the delivery of addictions services will be considered an asset
- Demonstrated ability to provide holistic and comprehensive case management services to all clients including initial and ongoing assessments, goal setting, short and long-term case plan development, crisis intervention, progress monitoring, individual money management,
- Knowledge of principles of case management theory and practice
- Demonstrated ability to use problem-solving techniques to identify and assess client needs and develop appropriate intervention strategies
- Demonstrated ability to engage in a meaningful relationship with a broad range of clients, including those who are resistant
- Skillful in assertive outreach and follow-up using a flexible approach to support/assist clients in remaining engaged
- Excellent knowledge of community resources and demonstrated ability to negotiate with others on behalf of clients
- Excellent communication and interpersonal skills
- Excellent clinical judgment and decision-making skills including
