Community Resource Navigator

Located in the heart of Southern Ontario, the City of Brantford is a rapidly growing community of over 100,000 residents. As a single-tier municipality, we are responsible for providing a wide range of municipal services. We are seeking a skilled Community Resource Navigator to join our team.

Job Summary

The Community Resource Navigator will play a key role in helping individuals and families access community resources, including healthcare, employment services, housing options, childcare, and other necessary supports. This is an in-person position based out of the Ontario Works Office in Brantford.

Key Responsibilities
  • Participate as a team member on the Community Resource Navigation Team
  • Provide navigation services to Ontario Works clients with complex needs
  • Support successful referrals to service providers in the community
  • Support clients to identify and attend appointments, assessments, and programs to support employment and well-being goals
  • Collaborate with internal and external partners and staff to ensure effective delivery and training related to designated services
  • Seek out resources and partnerships that benefit underserved and/or marginalized families
Qualifications
  • Minimum one (1) year post-secondary diploma/certificate in Community or Social Services or a related field
  • Minimum one (1) year of relevant experience working with at-risk and/or marginalized communities
  • Familiarity with municipal policies and procedures, as well as local community agencies and service providers, is considered an asset
  • Working knowledge of pertinent provincial and federal legislation and guidelines related to mental health, income support programs, employment, and training
  • Ability to travel to other locations in the community, as needed
  • Satisfactory Police Check Vulnerable Sector required as a condition of employment
