Job Summary

The City of Brantford is seeking a highly skilled and compassionate Community Resource Navigator to join our Community Strategies and Family Supports Department. As a key member of our team, you will play a vital role in helping individuals and families access community resources, including healthcare, employment services, housing options, childcare, and other essential supports.

Key Responsibilities

  • Participate as a team member on the Community Resource Navigation Team
  • Provide navigation services to Ontario Works clients, including those with complex needs such as mental illness, addiction, and/or homelessness
  • Support successful referrals to service providers in the community as necessary
  • Assist clients in identifying and attending appointments, assessments, and programs to support employment and well-being goals
  • Collaborate with internal and external partners and staff to ensure the effective and efficient delivery of designated services
  • Seek out resources and partnerships that will benefit underserved and/or marginalized families

Qualifications

  • Candidate must possess a minimum one (1) year post-secondary diploma/certificate in Community or Social Services or a related field
  • Minimum one (1) year of relevant experience working with at-risk and/or marginalized communities, including complex social issues such as poverty, mental illness, addiction, housing issues/homelessness, illiteracy, health issues, and concurrent disorders
  • Familiarity with municipal policies and procedures, as well as local community agencies and service providers, is considered an asset
  • Working knowledge of pertinent provincial and federal legislation and guidelines related to mental health, income support programs, employment, and training is considered an asset
  • Ability to travel to other locations in the community, as needed
  • Satisfactory Police Check Vulnerable Sector required as a condition of employment
