Overview:
**All About Seniors **is seeking a highly organized and compassionate individual to join our team as an LPN Case Manager to manage a caseload in Calgary, focusing on urban and rural south. As an LPN Case Manager, you will play a crucial role in coordinating and managing the care of patients. This role requires a motivated and organized individual that understands Senior Care and the important role Home Care Plays in the care continuum. We are a busy, family-owned and operated homecare and companionship company serving Calgary and surrounding areas since 1997. We strive to provide personalized, top-notch care to clients and their families and are looking for someone who aligns with our goals and visions.

This position involves speaking with family members who are searching for answers regarding their loved ones care needs. You will interact with a variety of different people regarding care needs and scheduling. Your experience as an LPN will help you to counsel and advise.

This dynamic role is responsible for effectively managing multiple demands that come with the changing needs of clients, while demonstrating a first-class experience. You will be working with our Director of Nursing/RN Care Manager to support managing your own caseload of clients and their needs.

This individual must be able to work well in a fast-paced environment with excellent prioritization skills. Must possess excellent communication skills, be client oriented with the ability to work well in a team environment. **You are required to have advanced English speaking and writing skills for this role**. Your health care background will help you understand the unique challenges our clients face and to be able to guide/support Health Care Aid staff as needed..

**Duties**:

- Coordinate patient care by scheduling visits, and ensuring necessary medical documentation is obtained
- Collaborate with Director of Nursing/RN Care Manager to develop, update and implement care plans for patients
- Provide support and education to patients and their families
- Maintain accurate and up-to-date patient records using electronic charting
- Communicate effectively with clients, their families, staffing and Director of Nursing to ensure seamless care coordination
- Required to drive to clients homes to provide teaching to staff and follow up on client concerns/potentially provide care to clients.

Qualifications:

- Previous Home Care experience preferred
- Experience working with geriatric populations is highly desirable
- Excellent organizational skills and attention to detail
- Strong communication skills, both written and verbal
- Proficiency in using electronic medical record systems such as Cerner
