Benjamin Residential and Vocational Options (BRAVO) is currently seeking a Home Share Coordinator for our Shared Living program. The Coordinator oversees contracted Shared Living providers who share their home with an adult with a developmental disability, who may also have behavioral challenges or unique medical needs.

This is a blended position which includes administrative support to the management team. This position reports directly to the Community Living Manager and works as part of a Shared Living coordination team, including the Agency Directors, the Community Living Manager, and Finance/payroll. This is a 35 hour/week position which requires a flexible schedule; it will be necessary to be available for early evenings to accommodate the needs of individuals who are away during the day.

The coordinator supports shared living providers with day-to-day planning, manages transitions, monitors services delivered, and provides quality assurance to our supported individuals, their families contracted providers, and our funder Community Living BC.

The Coordinator will monitor a caseload of 14 individuals living in ten locations. The monitoring is in line with standards established by Community Living BC and CARF—the Council for the Accreditation of Rehabilitation Facilities.

**Duties and Responsibilities**
- Implements and monitors CLBC Standards for Shared Living
- Prepares the Individual Service Plan in consultation with the person served and the service provider
- Plans and implements program activities and special events
- Oversees the day-to-day operations of the program to ensure that program policies, guidelines, and standards are in conformance with requirements
- Ensures that client-specific deadlines are met, appointments are scheduled and attended, and Individual Service Plan goal setting is implemented and followed
- Orients and determines the need for training of shared living providers
- Liaises with community social service providers to coordinate service provision
- Maintains records and statistics and produces reports as required
- Protects and upholds individual rights and confidentiality
- Performs other related duties as required

**Skills and Abilities**
- Excellent written and verbal communication skills to deal effectively with individuals, families, and stakeholders
- Excellent interpersonal effectiveness skills; proven ability to develop positive relationships with individuals, families, caregivers and involved professionals
- Demonstrated teamwork and collaborative skills
- Demonstrated organizational skills including the ability to maintain confidential records on Sharevision
- Demonstrated conflict resolution skills including the ability to make decisions and solve problems in high pressure situations
- Demonstrated ability to work independently and as part of a team, to take initiative, make sound decisions and to be held accountable for results
- Demonstrated ability to be sensitive and understanding of individual’s social realities, including race, gender identity, culture, religion, sexual orientation and lifestyle
- Demonstrated knowledge of issues and trends related to shared living

**REQUIREMENTS**

**Education and Experience**
- two-year diploma in a related human/social services field, or a degree in the Social Sciences
- two or more years related work experience, including a minimum of six months administrative/supervisory experience
- Satisfactory criminal record clearance
- Valid First Aid and CPR
- Use of a personal vehicle
- Valid province of BC drivers license
- Flexibility in work schedule as required and will involve some evenings; 35 hours/week
