**JOB** SUMMARY**: The Health Advocacy Worker reports to and works under the direction and supervision of the Manager of the Home & Community Care Program. Receives daily support from the designated Home & Community Care Case Manager.

The Health Advocacy Worker will provide support to navigate the health care system in accordance with current applicable guidelines and regulations, and may at times be directed by the Manager of Home & Community care, to assure that the highest degree of quality patient care can be maintained at all times.

**BASIC QUALIFICATIONS**:

- Minimum of Bachelor Degree in Social Work or Degree in related field **_OR_**
- Diploma in Social Service Worker with five years’ experience.
- Computer skills in Microsoft Office, MS Word and Excel an asset.
