EMPLOYMENT OPPORTUNITY

Housing Case Manager

35 hours/week

SUMMARY OF RESPONSIBILITIES:

- Assess families and individuals for strengths and barriers, including employment, behavioural health, criminal history and substance use and refer households to appropriate levels of housing and rental assistance
- Collaborate with the Intake and Assessment Worker and the client in the formulation of a comprehensive service plan that is reviewed and modified with client on a regular basis
- Provide outreach, case management, support, advocacy, and other needed services to clients in any environment including: the streets, shelters, prisons, hospitals, apartments, office, etc.
- Provide individually tailored services to each client - i.e. housing placement, independent living skills, vocational guidance, appointment escorts.
- Maintain written and computerized records, compile reports and complete other program documentation
- Assist clients to manage their monies including preparing budgets with clients and computing expenses.
- Cultivate relationships with landlords and property management companies serving low income families
- Share best practices as a means of building capacity within the Homelessness Network
- Work collaboratively with the Homeless Network partners in the delivery of services for homeless and at risk persons in the community, and the implementation of a Housing First system in the Sudbury community.
- Participate in case management meetings and processes as established by the Homeless Network partners
- Participate on and represent agency at relevant committees, events etc.

QUALIFICATIONS AND SKILLS REQUIRED:

- Post secondary education in social work or related field combined with relevant experience.
- Additional training in Crisis Intervention, Suicide Intervention, and CPR, First Aid, Non-Violent Crisis Intervention, Mental Health First Aid
- Direct experience working with individuals who have backgrounds of: criminalization, victimization, poverty, mental illness, addictions, homelessness.
- Extensive knowledge of community resources and programs, roles and functions of correctional centres, treatment centres, courts, probation and parole.
- Strong skills in areas of: case management, interpersonal, analytical, organizational, written and verbal communication, interpretation of and completing documents, statistical compilation
- tracking-interpretation.
- Vehicle required along with appropriate insurance coverage

**Salary**: From $22.79 per hour

Expected hours: No more than 35 per week
