**ABOUT MADISON**:
Madison Community Services is a non-profit agency, which promotes the independence, health, recovery and community integration of persons with mental health challenges through the provision of mental health case management, supportive housing and a broad range of other programs and services.

**DUTIES, RESPONSIBILITIES, AND RELATED TASKS**

1. Organizational
- Assist in the planning, implementation and evaluation of the programs and services.
- Comply with the agency policies, standards, guidelines and procedures.
- Participate effectively in staff and other designated internal meetings.
- Work in cooperation with management and other staff members in the provision of efficient and coordinated services.

2. Program and Services
- Carry out the client screening and intake process.
- Complete assessments that identify clients’ needs, priorities and abilities through information obtained from the client, his/her formal and informal supports and the referral source utilizing agency and funder tools.
- Develop with each client a comprehensive individualized plan, which incorporates his/her goals and values and identifies all skills, resources and service requirements.
- Provide case management direct service including: ADL skills coaching, assistance in obtaining and maintaining housing, support, encouragement, counselling and feedback to enable clients to reach their goals, counselling and information to reduce the incidence, duration, intensity and impact of problematic symptoms, family support and education as appropriate to support the interests of the client.
- In addition, provide direct service to clients related to the settlement process: assist in obtaining a permanent residence card; social insurance card, health card; help with registration in ESL programs; connection and referrals to community services specializing in serving newcomers.
- Provide feedback to the agency about client and community needs.
- Maintain confidentiality and comply with agency standards and all applicable legislation.
- Provide information and strategies for health and mental health promotion and crisis prevention.
- Assist clients in identifying and advocating for their civil, legal, housing and other rights.
- Facilitate access to desired and needed services and resources, including community supports, social recreational supports, crisis intervention and treatment, among others.
- Assist clients to identify, build and maintain a social/informal support system, when desired.
- Conduct mental health/settlement related workshops at the office or in partner agency’s locations, as required.
- Collaborate with community service providers.
- Participate in and contribute to internal agency committees.
- Perform duties in a safe manner, in line with Madison’s Health and Safety policies and all applicable statutes.
- May assist in the training and supervision of students and/or volunteers.

3. Monitoring and Evaluation
- Evaluate the achievement of goals (from the perspective of both the client and case manager) and client satisfaction.
- Monitor, regularly review and, if appropriate, revise priorities based on client’s individual needs.

4. Administrative
- Maintain client files and documentation as per agency and funder policies, procedures, standards and guidelines.
- Collection of data including: client’s demographics, service statistics, goal achievement and other systems that may be developed.
- Provide reports as required by the funder, IRCC.

5. Community Relations
- Maintain relations with networks, related professional organizations and other related service providers.
- Represent the mission, values and philosophy of Madison in the community including particular networks or meetings as requested.
- May assist in the training and supervision of students and/or volunteers

6. Professional Development.
- Set annual performance and professional development goals.
- Participate in regular supervision with supervisor.
- Attend relevant conferences, workshops and other in-service events as approved.
- Become fluent in Madison Policies, procedures, standards and guidelines of operation.
- Participate in agency internal consultations and committees as requested.

7. Other
- Performs other related duties as assigned by the Senior Manager of the Case Management Programs.

**QUALIFICATIONS, KNOWLEDGE AND SKILLS**
- Minimum 3 years of combined experience in the mental health and settlement sectors is an asset.
- University degree or equivalent credentials that are relevant to mental health & addictions case management is preferred.
- Strong understanding of recovery values and practices.
- A demonstrated understanding of settlement issues.
- Cross cultural communication skills.
- Strong computer skills.
- Fluent in Spanish and English.
- Strong written and verbal English and Spanish communication skills.
