The Coordinator Cancer Care provides professional services to patients within a designated tumour group or clinical trial, their families, and the community. In collaboration with members of the care team, the Coordinator provides emotional support, information, education, and patient care coordination to patients and their families as well as liaising with surgeons, oncologists, hospital and community partners to ensure continuity of care and to achieve optimal clinical outcomes and client satisfaction. The Coordinator develops standards and processes, monitors and evaluates services, and acts as a clinical resource for physicians, other health care professionals, and the community in matters relating to a designated tumour group or clinical trial. The Coordinator also performs duties supporting research.
What you'll do
Develop organizational policies, procedures, standards and processes for Cancer Care at the BC Cancer Agency (BCCA) in collaboration with the cancer care team for a designated tumour group or clinical trial; evaluates and monitors service outcomes and acts on results to revise and modify the program to improve services.
Provide leadership in the education of patients and their families by performing duties such as developing, implementing and evaluating teaching programs and educational materials. Acts as a resource to other nurses and health care professionals to support their role in teaching patients and works closely with members of other health care disciplines and physicians in coordinating patient education.
Act as a clinical resource for physicians, nurses and other health care professionals and students by providing both formal and informal education programs.
Coordinate care for patients referred to the BCCA by performing duties such as: assessing designated patients for educational and support needs and taking the necessary steps to ensure that the needs are met, including initiating referrals to other members of the team and other support resources; discussing clinical findings and providing support to the patients and their families as needed, in person or by telephone, before, during or after multidisciplinary clinics; assisting patients to understand diagnostics, surgery and treatment options; providing information and support to patients through their decision-making process regarding treatment options; advocating on behalf of the patient and family to support their choices and needs; following up with patients, evaluating and researching outcomes and evaluating treatment processes; documenting all teaching and supportive actions; and participating in patient care management conferences and other relevant multi-disciplinary clinical decision making meetings; coordinates testing procedures as required.
Support research activities for patients by performing duties such as: monitoring, recording, and reporting pertinent information to appropriate members of the team; identifying and evaluating the patient's eligibility for inclusion in clinical trials based on established criteria and assigning patients to treatment per randomization as required; and assisting physicians in explaining the tumour bank mandate and protocols and/or clinical trial as appropriate to patients and their families, expanding upon and/or clarifying information on the Patient Information and Consent form, obtaining the patient's signature on the consent form, and providing copy to the patient. Administers questionnaires as specified by research protocol and recording the data; submits progress reports to research directors, principal investigators and/or appropriate data centres. As required, performs duties such as accessioning for the purposes of acquiring blood and tissue samples for the tumour bank and performing all tasks associated with the handling and shipping of samples to a Tissue Processing and Storage Laboratory and coordinates activities of the tumour bank in accordance with the Tumour Tissue Repository's Standards of Protocols.
Coordinate and completes data collection by performing duties such as obtaining information from the patient; developing data collection tools; maintaining records and current data on patients and procedures performed using a computer where necessary; and assisting with external audits.
Collaborate with members of the cancer care team to identify gaps in cancer services and develops strategies to enhance the community's capacity to care for patients. Attends group meetings.
Promote the outreach in the community by performing duties such as liaising with the BCCA library and encouraging the use of the library as a resource for information; responding to requests for formal and informal education sessions from health care providers, community groups and agencies; networking with agencies such as the Canadian Cancer Society and participating in committees to enhance Cancer Care; and educating the public, community organizations and other health ca
