The Seniors’ Health and Ambulatory Care program is recruiting a full-time, permanent Patient Care Manager for its Specialized Geriatric Services, and Adult Day Program at Providence Healthcare. This opportunity will be of interest to leaders who value and foster a senior friendly approach to care. In addition, being results orientated and comfortable working in a fast paced post-acute care setting is required.
You must be able to work using a collaborative, open and client-centered approach with patients, families, community stakeholders and inter-professional staff. This role actively engages with external stakeholders (e.g. Regional Geriatric Program of Toronto) to advance the delivery of seniors’ programs and services at Unity Health Toronto.
The Patient Care Manager, Specialized Geriatric Services and Adult Day Program, promotes and facilitates senior friendly care inclusive of evidenced-based care under the direction of the Senior Clinical Program Director, Seniors’ Health & Ambulatory Care. This will be accomplished through the implementation and use of quality improvement, utilization management, risk management, professional practice standards, staff development, information systems, outpatient and community services benchmarking targets and resource/supply chain management.
**MAJOR RESPONSIBILITIES**:
- Accountable for maintaining efficient, effective operations within a defined fiscal envelope and day to day management while ensuring professional practice and staff development are in keeping with changing needs;
- Promotes best practices and evidence-based care through the implementation of initiatives that are aligned with strategic directions of the organization;
- Responsible for ensuring that roles, responsibilities, functions and care delivery models continue to evolve to meet changing patient/client, family, departmental and corporate needs;
- Develops and evaluates quality improvement initiatives in focus areas;
- Participates and collaborates in planning, developing and delivery of programs and services; accountable for targets and outcomes;
- Provides leadership to the inter-professional team in order to ensure the holistic needs and preferences of individual patients/clients are assessed and guide care planning;
- Works closely with the Senior Clinical Program Director, Seniors’ Health & Ambulatory Care, and Medical Director, Seniors’ Health, to ensure quality programming;
- Maintain direct communication and collaboration with patients/clients and families to ensure participation in and satisfaction with care;
- Liaise with external and internal referral and care partners to ensure that outpatient, outreach and community services and processes are well managed;
- Maintain a well-established relationship with the Adult Day Program Operations Lead, Inter-professional teams and a good rapport with other departments within the organization;
- Ensures that an integrated, effective plan of care is developed, implemented and evaluated;
- Seeks, communicates and implements geriatric rehabilitation best practice and senior friendly care approaches;
- Implements methodological approaches to achieving improvements in patient/client access and flow; works collaboratively to ensure seamless care across the Seniors Health & Ambulatory portfolio; and
- Participates actively in Program specific and Hospital-wide initiatives through Committee and Council membership, and Hospital planning and management forums.
**QUALIFICATIONS/SKILL REQUIRED**:
- Clinical background with current registration in good standing with a Regulated Health Professional College;
- Master’s Degree preferred;
- Three to five years strong clinical and/or leadership experience in seniors’ health services in a health care setting providing care/service for older adults;
- Strong leadership skills including demonstrated effectiveness with designing, planning, managing and change management;
- Demonstrated ability to foster partnerships to advance patient/client and family-centered care;
- Demonstrated evidence of innovative leadership and strong physician engagement skills, with proven ability to foster a collaborative team;
- Superior communication, conflict resolution and problem solving skills within a complex and dynamic resource environment;
- Demonstrated excellent budgeting and financial management skills;
- Experience in facilitating, collaborating and negotiating effective resolution of human resource issues;
- Demonstrated human resource management skills including recruitment and retention of staff;
- Demonstrated strong commitment to patient/client safety, best practice and innovation in patient/client care delivery;
- Must be able to multi-task, set priorities in a fast-paced setting;
- Excellent record of attendance and punctuality; and
- Knowledge and understanding of LEAN methodology an asset.
