**ROLE PROFILE**:
**Position**: Journey Program Manager
**Reports to**: General Manger
**Location**: Residence
**Scope**: The Journey Program Manager supports the overall philosophy of the program that reflects a person centred approval to addressing resident’s needs. The Journey Program Manager’s role is to create partnerships among the Retirement Residence teammates, the residents, and their families, incorporating external stakeholders where applicable, with the goal of creating the best possible outcomes that will enhance the quality of life and quality of care possible for those with dementia.

**Key Responsibilities**:

- Overseeing the day-to-day operations to include financial cost efficiencies and provision of quality care and quality of life services to residents in the Journey program.
- Model and incorporate Person Centre language to show respect and dignity when interacting with others and continually monitor and correct, in a respectful manner, when the use of inappropriate language is used.
- Support teammates to further their dementia education and skills by teaching Gentle Persuasive Approach (GPA)
- Include teammates in decision-making and provide opportunities for them to give feedback or express their concerns.
- Encourage teammates to try new approaches and offer suggestions that will enhance resident’s quality of life
- Integration of care and services that are person-centered and incorporates the personal preferences, values, lifestyle choices and needs of the resident in their service plans
- Determining the stage of dementia (Mild, Moderate or Severe), the resident’s cognitive and physical functional assessments, service care planning, medication management, behavioral interventions, and therapy for activities of daily living needs.
- Develops an individualized resident engagement calendar that offers structured and personalized activities that will assist in reducing agitation, improving mood and enhance the quality of life for the resident.
- Develop and pilot new innovative resident experiences and programs
- Leverage technology to develop, enhance, promote, and provide in-suite and group programming within the community to engage residents.
- Certified to teach group fitness to seniors and or willing to obtain within 60 days of employment. Required to maintain certification through annual continuing education training provided by Aspira.
- Facilitate group programs and experiences that reflect the domains of wellness and adapt to meet residents’ diverse abilities and needs.
- Plan and facilitate department team meetings. Meetings should be held, at minimum, monthly for general department operations and quarterly to plan larger community initiatives, such as special events and holiday celebrations
- Develop and maintain a quality committee that will focus on meeting the standards

**Education & Experience**:

- A degree/diploma in Nursing, Gerontology, Kinesiology, Therapeutic Recreation, Health Promotion, and Health Sciences.
- Minimum 3 years’ experience in Geriatrics, Rehabilitation and/or Retirement industry.
- Trained in GPA, PIECES and/or U-First, ability to train others
- Must be able to set priorities and meet time and calendar deadlines as workload may fluctuate as resident needs and occupancy change
- Effective verbal and written English communication skills required
- Effective interpersonal and communication skills to establish and maintain strong relationships with internal and external stakeholders.
- Must possess excellent supervisory and leadership abilities.
- Strong computer skills in Microsoft Office, Excel, Outlook.
- Strong analytical, problem solving and decision-making skills.
