**Position Description**:
The Patient Care Information Coordinator, Hemodialysis Program, will be responsible for patient scheduling processes and facilitating the delivery of patient care by providing a high level of support and organizational assistance to program staff and patient/families. They will support the operation of the team by providing clerical and administrative functions in a proactive and efficient manner. This role will be primarily responsible for all scheduling and coordinating activities for patients coming for treatment in the Dialysis Apheresis Unit and for appointments in the Home Dialysis Programs. There is significant interaction across many departments at SickKids to coordinate care. The coordinator also supports clinic flow during clinic operation by ensuring smooth registration and service delivery. The patient care coordinator also is a main contact for billing and other administrative functions to support the operation of the programs.

**Here's What You'll Get To Do**
- Respond to requests for information from families and healthcare staff
- Greet and orient patients and their families to the program/clinic
- Ensure related scheduling requests are processed; reschedule appointments as necessary and provide updates to the clinical team
- Facilitate communication flow via telephone, paging, and computer access
- Maintain appointment schedules in Epic and notify patients/families of any updates or changes to their appointments; facilitate coordination with other departments at SickKids and occasionally at UHN
- Support the scheduling of clinical rounds; update the on call schedule for nurses
- Maintain an efficient and tidy work environment
- Responsible for ordering of supplies. initiating accounts for billing for home patients, facilitate internal billing inquires

**Here's What You'll Need**
- Health-related post-secondary education (Medical Administration, Business Administration, Health Administration) preferred
- Minimum of 2 years clerical or administrative experience in a health care setting. Experience in a hospital environment is preferred
- Scheduling experience is essential and medical terminology is an asset
- Accurate keyboarding skills and strong working knowledge of Word, Excel, PowerPoint, and Outlook
- Experience with Epic is an asset
- Excellent problem solving skills that demonstrate an ability to anticipate potential issues and make suggestions
- Strong interpersonal, communication and organizational skills with both fellow colleagues and patients
- Ability to remain focused and prioritize tasks in a busy and high stress environment
- Dependability and good attendance
- Demonstrated commitment and actions in advancing equity, diversity, and inclusion objectives

**Employment Type**

1.0 FTE, permanent employment; Monday to Friday 0700-1500
