**about the role**:
The Corner is a multi-service, inclusive hub governed by residents and service providers and is a unique, integrated service model between 20 partner organizations. It is the primary hub for HASJT intake, referrals and service coordination and is aimed to enhance interdisciplinary care including primary care, improving access to health, social and community services in St. James Town. Thorncliffe Neighbourhood Office and Sherbourne Health co-lead this integrated service model comprising The Corner and HASJT.

You are a team player who is an integral member of The Corner, greeting clients and ensuring they feel welcome and are provided with information about The Corner and partners agencies’ services. You support and provide culturally sensitive services with an anti-oppressive framework. You will be working in a fast-faced environment with a multidisciplinary team to provide services.

**day to day responsibilities**:

- Registration of all new clients at The Corner and in the community, satellite sites to ensure the client navigates and access the services with ease in a safe and respectful manner.
- The position also works closely with the Holistic Intake & Navigation function at The Corner and updating information on new/existing clients in the database and clarify eligibility criteria.
- Engage proactively with residents, clients and service users, and will be responsible for supporting staff and partners with crisis intervention and de-escalation as needed.
- Good organizational skills to ensure a smooth flow of clients to the different services on site and to work closely with the intake staff to ensure all residents seeking services are connected and directed as appropriate.
- Keep up-to-date information on existing and new services, activities, and eligibility criteria for partner agency services available at The Corner
- This position requires the ability to work flexible hours, including evenings and weekends
- Develop and maintain cooperative relationships with partner agencies and service providers at The Corner
- Demonstrate a commitment to a healthy and safe workplace for self and others (staff, patients, families, etc.) by taking all reasonable precautions and working

**To thrive in this role, you’ll need**

**Education/experience**:

- A Social Service Diploma and/or equivalent formal training and 1 - 3 years’ experience working in a community health setting or equivalent blend
- Understanding and experience in the harm reduction framework
- Experience working with newcomer and diverse populations, including individuals with mental health, addictions, isolated seniors, hard to reach, high needs and adults with special needs and the 2SLGBTQ population
- Involvement in outreach activities
- Experience working with populations facing multiple barriers to equitable care
