Peer Support Specialist

Job Profile:
- **Maintain a high-level of competency in Electrical Medical Records, scheduling, information entry and assessment, data management, and document management.**:
- **Transfer Data from the Digital Front Door platform to our EMR.**:
- **Communicates with Clinic Coordinator, MRP, Pharmacy and Lab Technician around patient needs, change in status and all emergency situations.**:
- **Complete intakes and facilitates client care using virtual telemedicine**:
- **Responds to general inquiries and makes appropriate referrals within the RAAM Team.**:
- **Assist with facilitating in-person and virtual treatment to patients in marginalized housing, shelters and COVID isolation shelter to support the health and well-being of BHN RAAM Clinic clients in the Brant Haldimand and Norfolk Area.**:
- **Point of Care Urine Drug Screens**:
- **Encounter information and referral appointments, answer phone calls, client follow-ups, outreach, and group activities into the BHN RAAM Clinic’s Electronic Medical Record.**:
- **Other tasks designated by the RAAM Clinic Coordinator**:
- **Requirements**: A degree or diploma in the health or social services, or a personal support worker

- Experience in the mental health and addiction
- Experience in mental health and/or addiction recovery.
- Ability to observe boundaries, engage in appropriate emotional regulation, refrain from dual relationships with consumers, maintain confidentiality, and engage in reasonable self-care strategies designed to reduce stress by balancing work/life responsibilities.
- Ability to prioritize needs and organize work efficiently.
- Ability to work in
- Aptitude in developing innovative approaches to service delivery and working in a chaotic environment.
- Excellent oral and written communication skills
- Ability to deal effectively and calmly with challenging situations.
- Excellent problem-solving and decision-making skills.
- Knowledge of the Ontario Mental Health Act, mental health reform principles, the Substitute Decisions Act, the Health Care Consent Act and PHIPA requirements
- A valid Canadian driver’s license,
- A satisfactory Vulnerable Sector Screening or Police Check

Schedule:
- Monday to Friday
- Weekends as needed
