**Position Summary**

The CASE WORKER will play a pivotal role in providing support, advocacy and guidance to children, youth, and their caregivers, through the delivery of various programs within the Family Wellness Hub. The CASE WORKER will support such programs as, Family Check-Up (FCU), Everyday Parenting (EDP), Collaborative U14 Intervention Program (CUIP), and will assist with system navigation and access to resources.

The CASE WORKER will also provide frontline support through the Trusteeship Program, in partnership with Northumberland County. This program provides services to recipients of various community and social services programs including Ontario Works, Learning, Earning and Parenting (LEAP), Housing, and Homelessness. The Trustee has a responsibility to manage either all, or a portion of the recipient’s income and is expected to perform the role of a fiduciary-executing all necessary powers and duties diligently, honestly, with integrity and in good faith.

**Roles and Responsibilities**

Case Management
- Provide case management to clients and their families upon referral to programming & services
- Refer and link clients to appropriate services and programs, both to Rebound and external community supports
- Preparation of assessment materials that include relevant data
- Maintain client files
- Database entry and recording of information for referred and participating clients, including for purposes of program evaluation, assessment of needs and statistical reporting
- Research supportive services and programs
- Attend meetings with schools, landlords, health care providers and other social service

providers as required
- Participation in committees as arranged with the Executive Director
- Any other duties as assigned and/or requested by the Executive Director

Trusteeship
- Assist students under 18 to access Ontario Works through Northumberland County
- Perform intakes and assessment of needs of designated case files, to identify concerns and barriers to financial stability, including; housing, budgeting, education, employment, addiction, health, nutrition, relationships, basic life and living skills and mental health and the development of possible solutions
- Assist clients with financial management, including; bill payments, housing stability, and budgeting
- Review all individual trusteeships monthly - record and report client income to OW case worker
- Maintain accurate financial records

Family Wellness Hub
- Perform intakes and assessment of needs of designated case files, in collaboration with clinical team members
- Provide case management to families in the Family Wellness Hub
- Development, facilitation, and coordination of the Family Wellness Hub Programs
- Facilitation of case conferences with families and multiple community agencies and supports; this may occur in person or via videoconferencing
- Support the clients’ achievement of safe, realistic, and reasonable goals within a complex

health, social and fiscal environment
- Outreach and liaison with local police services.

Mental Health Services (MHS) Team
- Provide support as required on the MHS Team, reporting to MHS Team Lead
- Implement skills building plan of care as directed by MHS Team Lead which may include: specific, short term support and follow-up, including the use of cognitive behavioural approaches, goal setting, problem solving and helping define priorities as necessary; supportive discussions with youth and families/caregivers, and addressing service options (e.g. mental health, recreation, health, education, etc.)
- Act as advocate and liaison for eligible youth and/or their families in relation to accessing supports and programming in school and/or in the community at large
- Update, maintain and authenticate case notes and files weekly
- Group program facilitation

**Working Conditions**
