Reporting to the Manager of the Assertive Community Treatment Team (ACTT), the Social Worker will work in a multi disciplinary team of health care professionals comprised of a Program Psychiatrist, Nurses, Peer Support Worker, Vocational and Addictions Specialists, Mental Health Clinicians, Social Workers, and an Occupational Therapist. These individuals provide intensive treatment, rehabilitation, and support services for people 18 years of age and over with severe mental illness.

The Social Worker is responsible for working with the clients and their families in the community on the Mississauga Assertive Community Treatment Team (ACTT). The Social Worker provides supports for individuals experiencing mental health and/or problematic substance abuse challenges and are at risk of homelessness.

**General Duties and Responsibilities**:

- Counselling and educating families about severe and persistent mental illness.
- Interviewing individuals, families and significant others to assess clients’ social, emotional, physical and mental impairments, as well as financial needs.
- Assisting clients in obtaining appropriate community resources for financial support, such as ODSP, Ontario Works, supportive housing, etc.
- Leads group counselling sessions and plans psychosocial programming for clients in need of specific services.
- Obtaining a thorough social history in a timely manner with each new client and bringing relevant concerns to the attention of all team members.
- Individualized psychoeducation for families about the client’s illness and the role of the family and other significant people in the therapeutic process.
- Supporting client self-determination in relation to legal matters (charges, diversion, probation, CTOs, SDMs, PGT- guardianship, POA, Mental Health Advance Directives and PHIPA).
- Sharing of shift work including weekends and on call duties with other ACTT Staff is also required.

**Qualifications**:

- Bachelor’s degree in social work and must have registration with appropriate professional association
- 3+ years’ experience working with adults living with serious mental illness
- Strong communication skills, assessment, risk management and crisis de-escalation
- Proficient in MS Suite (Word, Excel, Power Point and Outlook)
- Use of a vehicle on a daily basis and valid driver’s license
- Valid Certification in First Aid & CPR
- Training in ASIST and CPI
- Vulnerable Screening Level 3

**Working Conditions and Physical Requirements**:

- Typical shift is Mon-Fri 8:30am-4:30pm but every couple of months will be expected to work evening shift from 11am-7pm which includes working on call
- Able to work well with internal and external clients
- Able to sit for prolonged periods of time
- Able to answer and communicate telephone calls and general computer use
- Computer screen use (multiple screens if applicable) for prolonged periods of time
- Able to lift up to 20lbs (product, documents, etc.)
- Travel by vehicle to community sites for client support as required
