JOB SUMMARY


Social workers employed in the THP cancer program work in a fast paced environment as collaborative team members of the interdisciplinary team completing assessments, providing evidence based interventions, developing programs and managing programs and projects.

Social workers provide bio-psychosocial assessments pertaining to the biological, psychological, and social consequences of diagnosis, treatment and/or hospitalization as a result of as a result of cancer and/or issues related to current or historical stressors.

Social work assessments and treatment plans are goal-oriented for individuals, families, groups and couples to enhance the coping and resources of patients, families and/or caregivers.

Interventions are facilitated through counselling, psychoeducational and therapeutic groups, advocacy, identification of barriers to treatment, resource referral, consultation, collaboration, teaching, research, and patient/family/staff education.

Provision of social work services is in accordance with the mission, philosophy and values of Trillium Health Partners, the standards of the Ontario College of Social Workers and Social Service Workers.

As well, social work activities are provided in accordance with associated legislation and in collaboration with Cancer Care Ontario and Canadian Association of Psychosocial Oncology.


MAJOR RESPONSIBILITIES
To assess the psychosocial needs of the patient/family affected by cancer and treatments.

This assessment utilizes evidenced based practice, clinical judgement and standardized assessment tools to accurately reflect and assess:

- the patient's perception of the problem and their goals
- risk/safety concerns
- need/eligibility for community services
- living situation and functional status
- barriers to treatment and/or discharge
- level of distress and coping strengths
- patient's network of supports
- financial concerns
- capacity for decision making

  • To cocreate, with the team, patient and family, a treatment/discharge plan based on the assessment and the goals of the patients, families and/or caregivers.
  • To document these assessments and treatment plans in accordance with hospital standards as outlined by the Manager of Psychosocial Oncology, the Regional PSO Lead, the Regional Psychosocial Oncology Lead, the and Manager of discharge planning and the College of Social Workers and Social Service Workers.
  • To collaborate with the interprofessional team to address ongoing patient needs, to participate in regularly scheduled team rounds and ad hoc discussions to help optimize patient care, hospital bed flow and to ensure the coordination of community based support services.
  • To ensure patient, family, and/or caregiver participation in the decision making process related to their medical care plan and decisions regarding the continuing or ending of active treatment, and to provide advocacy on behalf of the patient, family and/or caregiver in order to ensure that the resources and supports which they require can be accessed.
