The role of the Family Wellness Permanency Coordinator is to work with children/youth under a Permanent Guardianship Order legal status and their caregivers to achieve permanency goals, under the Collaborative Service Delivery (CSD) mandate. The Permanency Coordinator’s role includes supporting lifelong connection with children/youth’s family and culture of origin. This person would also be supporting the legal permanency process for some children and youth working towards private guardianship or adoption.

You will be responsible for providing the following services:

- Case management in partnership with Child Intervention Practitioners from Children and Family Services (CFS - Fort Saskatchewan).
- Front-line placement stabilization, support, and preservation work
- Completing family maps/genograms and supporting lifelong connection with the child’s family and culture of origin
- Orientation of children, youth, and families to the CSD permanency process
- Coordination between caseworkers, family wellness coordinators, court workers, etc. and caregiving families/placements; and provision of CFS and legislation-related information to families/placements as necessary
- Coordination of all financial entitlements for children, youth, and families as necessary
- Coordination of additional services, such as respite care, counselling, and recreation opportunities, to support excellent outcomes for children and youth in care
- Advocacy for children, youth, and caregivers that ultimately supports permanency and lifelong connections for all children in care
- Participation in Family Group Conferences and Circles
- On-call supports to families, colleagues, and CIIRT (after-hours Children and Family Services) for one weekend approximately every 20 weeks
- Critical thinking and creative problem solving as a means of providing high-quality services
- Completing detail-oriented, time-sensitive documentation
- Supporting Indigenous children, youth and families
- Excellent conflict-resolution skills and the confidence to take constructive feedback and in turn motivate others to the accountability of legal permanency in our work
- Understanding and adhering to complex legal permanency processes, and educating others (children, families, and colleagues) about these processes
- Collaborative case planning and coordination of integrated services
- Working with contemporary practices, including but not limited to: Signs of Safety, Appreciative Inquiry, Neuroscience, and Trauma Informed Practice
- Contributing to the successful development of the Chimo Family Wellness Program

Physical Requirements:

- You may be required to work day, some early evening and the occasional weekend to ensure we are centering our practice around family's availability (this means you're available from 8:30am to 4:30pm M-F and will be selected for back up On-Call shifts 3-4 times a year; which caters to working evenings/night shifts).
- The work requires the ability to carry car seats, lift children in/out of cars and car seats, etc.
- You must be able and prepared to physically interact with children in both indoor and outdoor settings.
- Outdoor engagements with children and families may occur in all seasons, with appropriate measures taken based on current weather conditions.

Education and Credentials:

- College Diploma/Degree in the Human Services field (Child & Youth Care, Social Work, etc.)
- Minimum two years of experience in a front-line role within Human Services.
- Valid Class 5 Driver’s License
- Access to a reliable vehicle (note that while Chimo does have a small fleet of vehicles for staff use, use of a personal vehicle is required more often than not).
- Vehicle insurance with a minimum of $2 million PLPD and business use coverage.

- $52,035.00-$53,235.00 per year
