As part of the Toronto Community Hepatitis C Program (TCHCP), the Health Promoter-Hepatitis C works collaboratively with interdisciplinary care teams and partner agencies to provide HCV treatment and support for people who use drugs and/or alcohol, have mental health challenges and face other barriers to accessing health care. The aim is to improve people’s quality of life by reducing the barriers and stigma for people living with Hepatitis C, providing equitable access to comprehensive care and services, strengthening the capacity of people with lived experience of HCV to self-advocate, and creating a sustainable program in order to build a healthy community.

The Health Promoter-Hepatitis C position operates within SRCHC’s harm reduction services mandate. This position will work collaboratively with other team members, service providers, clients and community in the identification and resolution of community health issues and integration of services.

This position is expected to work onsite.

**Responsibilities**:

- Provides comprehensive HCV/harm reduction services within a social-determinants of health and human rights focus, utilizing health promotion, community engagement, community development, social justice, trauma-informed and anti-racist/anti-oppressive (ARAO) strategies
- Actively outreaches to marginalized community members with HCV in order to bridge to program and treatment teams
- Network with internal and community partners to ensure an optimal continuum of health & social services through all stages of HCV education, support, care, treatment and research as well as overdose prevention
- Provides information and referrals to health and social service supports
- Collaborates with TCHCP partner programs, healthcare partners, SRCHC programs and other HCV teams across the City of Toronto
- Collaborates with research partners and participates in research activities as required
- Participates in training volunteers and students, including many with lived or living drug use experience as well training and knowledge transfer activities for the TCHCP and SRCHC programs
- Participates as a team member in various team functions and meetings
- Follows and supports the maintenance of established protocols throughout the TCHCP programs, ensuring clear, accurate and timely written documentation in accordance with established procedures and policies set by the Ministry of Health and SRCHC
- Routinely compiles and enters data as per program directives and organizational requirements
- Prepares statistical data, funding reports and other documentation, as required
- Identifies health related education needs and plans in consultation with community members and develops, implements and evaluates health education programming - using various formats such as community events, group programming, use of media and presentations to large groups
- Disseminates accessible health education materials, developed with community members
- Uses a health promotion approach to assist the east Toronto community by raising awareness and advocating for appropriate diabetes programs and service for our priority communities
- Collaborates and builds partnerships around issues of concern to effect change
- Collaborates within own team and other teams across the organization to identify and provide health education to target groups
- All other duties as required

**Qualifications**:

- Post-graduate degree from a recognized university in health, social science, adult education, or a related discipline, or a combination of undergraduate degree and significant experience
- 3-5 years health promotion/education experience in a community-based organization
- Excellent group facilitation skills, experience and knowledge of health promotion and education, community organizing techniques and program design, implementation and evaluation
- Knowledge and experience in social prescribing
