**LOCATION**: This position is to cover Burlington and the Niagara North West region in Ontario.

Provides administrative and professional leadership, mentorship, supervision, attendance management, practice coaching, evaluation and coordination of all sub region BSO LTC staff supports and scheduling within the Long-Term Care Homes. Ensures the practice of staff employed by St Joseph’s Villa in the BSO program is professional and meets the required practices of the program and HNHB area and the programs RNAO Best Practices Spotlight Organizations mandates.
Manages the referral process and prioritizes and allocates clinical responses based on BSO model for care pathways. Coordinates and supports a collaborative effort for the safe, effective transition of residents across health care sectors.
**DUTIES AND RESPONSIBILITIES**
**Staffing**:
1. Participates in the orientation of all new hires to the sub region.
2. Identifies gaps in services, training and performance, in collaboration with the BSO Leadership Team
3. Maintains the master schedule and database for the sub-region in accordance with BSO practices to ensure fair and equitable coverage of the Long-Term Care Homes (LTCH) with a focus on frequency of supports, based on referral rates and consistency of staff assigned.
4. Manages sick calls, personal days, vacation requests, LOA, MLOA requests in accordance with St. Joseph’s Villa practices and reports same daily by 1 pm to the HR Lead/ delegate, for time cards.
5. Reviews and authorizes travel expenses for the team members prior to submission to the program Senior Manager.
6. Ensures regular communication with the LTCH’s leadership team to evaluate staff efficacy, performance and needs.
7. Oversees weekly staff scheduling to ensure that hours are based on care needs of the residents served and for patterning of hours.
8. Models and coaches positive intent, leadership skills, collaboration and professional boundaries in all interactions with the BSO staff, health care partners and LTCH leadership teams.
9. Regularly and routinely mentors staff in the field, working along-side, collaborating with and supporting with referrals. Managers are required to complete a minimum of one evening and one weekend visit per month.
10. Offers support to the BSO staff and LTCHs within scope of practice, with a focus on capacity building and collaboration.
1. Ensures privacy and confidentiality is maintained in all BSO practices in accordance with St. Joseph’s Villa, the host employer’s policies, PHIPA and regulatory colleges.
2. Manages the performance of staff by investigating complaints and performance concerns. Complete site visits for planned and unplanned audits and follows the organizations Performance Management and Disciplinary process, including Performance Improvement Plans, in collaboration with the Senior Manager.
3. Advises the program senior manager immediately of any serious performance violations or privacy breaches.
4. Attendance management of all staff in the sub-region in accordance with organization practices; Participate in supportive meetings, coaching, offer EAP, and identify trends. Collaborate with Senior Manager as needed.
5. Tracks staff attendance at team meetings and educational events.
6. Works as a front-line staff member as needed to manage referrals in LTCH where staff are unavailable or where staffing shortages prevent the timely progression of a referral.
7. Supports staff education through mentorship and coaching, in accordance with program standards and RNAO Best Practices.
8. Ensures only approved practices and tools are used by staff; assessment tools, documentation and non-pharmacological approaches
9. Ensures CUPE collective agreement is followed
10. Supports other managers as needed, with vacations and vacancies.
**Leadership and Engagement**:
1. Submission of data collection monthly to meet BPSO designation requirements.
2. Submission of data collection quarterly to Strategic Leads/Ontario Health to maintain MSSA requirements.
3. Participates in engagement activities, educational events, mandatory training
4. Attends quarterly meetings with external partners to share information about BSO program, themes and trends and build relationships. Attend scheduled program meetings, virtually or in person as required
5. Participates in committees, engagement activities and working groups, as required
6. Participates in self
- assessment and ongoing education as assigned to maintain and develop leadership skills
7. Identifies gaps, themes and educational needs of program staff in collaboration with the BSO Leadership team
8. Promotes and implements all BSO activities and material circulation as advised by the Senior Manager, the BSO Strategic Lead and the BSO Advisory Committee as required by Ontario Health.
