Registered Practical Nurse, Outreach Position (40 hours/week, Permanent) Who We are:

What We are Looking For:
We are currently recruiting for an Registered Practical Nurse to join our Interprofessional Primary Care team for a full-time position. This role is responsible for providing clinical care for vulnerable complex individuals and families, including those who are precariously housed, dealing with addictions, substance use disorder and/or mental health concerns. This position is based out of Langs main site, however frequent travel to outreach clinics, including the drop in at 150 Main in Cambridge, will be required.
- Registered Practical Nurse diploma from a recognized institution.
- Certificate of registration from the College if Nurses of Ontario
- RPNAO membership
- Three to five years nursing experience in a community setting or combination of community and hospital or public health settings; experience in a primary care setting, an asset
- Able to perform footcare, advanced footcare training skills preferred.
- Experience in program development, implementation, monitoring and evaluation, an asset
- Ability to work one evening per week (12:00pm-8:00pm)
- Valid G-Class License and a reliable vehicle to travel throughout the Cambridge & ND area.
**In this role you will be expected to complete the following tasks**:
- Perform triage function both in person and by phone.
- Check lab results and provide appropriate follow-up.
- Assist clinical providers with clinical and administrative procedures including but not limited to:
- Provide care both on site and at partner locations.
- Venipucture as ordered.
- Footcare
- Provide injections as ordered.
- Assist with cryotherapy for wart removal.
- Ear syringing as ordered.
- Remove sutures.
- Point of care testing as required (BS, urine drug screen, pregnancy testing) Perform other clinical activities according to the College of Nurses of Ontario standard of nursing practice
- With a doctor’s order provide immunization for people of all ages
- To assist with the organization and implementation of vaccine clinics at outreach sites. Including ordering vaccines, cold chain, and liaising with public health.
- Assist with tasks that facilitate patient flow i.e. bringing patients in, helping patients obtain specimens, helping patients prepare for examinations, supporting patients through examination.
- Maintain outreach clinics: this may include maintaining the medication inventory, resources, materials and equipment. Ordering medical supplies, arranging for bio-waster pick up, and specific examination/treatment trays.
- Have an understanding of infection control practices in a primary care setting, and implement best practices in a variety of settings in order to reduce the opportunities for infection and/or contamination.
- Document and encounter all patient contact/treatment according to the CHC policy and professional standards.
- Participate in IPC Team activities and projects including case review and chart audits, discussion, special clinics and screening programs.

- Starting Salary of $53,178 -$63,135 annually.
