**Indigenous Case Manager - One Roof**

**Overview**
The Indigenous Case Manager - _One Roof _program assists clients to navigate services within One Roof, especially high-risk youth engaging in criminal behavior. Case managers will work primarily with Indigenous participants to reduce barriers, increase motivation, and reinforce treatment plan recommendations.
**Description**
The
- _Indigenous _Case Manager - One Roof is* responsible for:
a) Supporting a program match that has the most chance of success for that client by:
- conducting the initial intake and ensuring that clients are aware of the One Roof Program and all accompanying resources, policies, and expectations;
- making referrals to appropriate community resources whether internal or external, especially services that are culturally relevant;
- providing warm transfers to the clinical team and other resources where necessary;
- utilize active listening and motivational interviewing skills from a traditional perspective to discover client’s individual strengths, barriers, risk, and protective factors;
- initiating and/or reiterating a discussion on what it means to be treatment ready for those youth who are eligible for the Crime Prevention Program, and providing on-going support to ensure barriers are removed or plans are put in place to minimize impact of individual barriers;
- Collaborates with One Roof Elder to ensure Culturally Relevant Services are provided;
- b) Providing on-going case management supports with the prime objective to maintain client participation by:
- retaining participants through developing rapport and trust to decrease individual barriers and stigma associated with accessing supports;
- effective problem-solving responsivity to help youth with a variety of issues including but not limited to homelessness, income supports, harm reduction, basic needs, and a lack of meaningful belongingness; accompanying the client to assessments, meetings, case conferences etc. when requested by client; maintaining contact with the treatment team to ensure that there are no issues impacting program participation;
- engaging/ reengaging clients to the program and other supports by tilizing assertive engagement techniques and community outreach;
- helping clients follow their individualized treatment plan as put forth by the Clinical Treatment Team;
- assessing risks presented by the client and develop appropriate plans to help ensure continuation of services and mitigate risk;
- creating wellness plans in cooperation with mental health and/or allied professionals and assisting clients in incorporating the wellness plan activities;
- c) Engaging the client with family supports by:
- reconnecting client to their family supports when requested by client;
- connecting client’s family to the clinical team in order to have them participate in the program in order to benefit both client relationships and outcomes;
- provide ongoing support to client’s family when appropriate;
- d) Engaging the client with community supports by:
- linking the client to meaningful daily activities as identified in the individual service plan so that the client experiences community integration and supportive social networks. All reasonable efforts are made by the support workers to connect the individual with the resources necessary to optimize success;
- being knowledgeable about the network of community services, supports, and resources in the Indigenous Community;

**Required skills and Knowledge**
Knowledge of criminogenic needs and the ability to help clients build on protective factors and mitigate risk factors.
- Knowledge of and experience utilizing Traditional Indigenous Healing Practices.
- Knowledge and experience with of various Indigenous Cultures.
- Knowledge of Residential Schools and Intergenerational Trauma
- Knowledge of and experience implementing Culturally Relevant Supports and Programming.
- Knowledge of Local, Provincial, and Federal organizations that support Indigenous Youth.
- Knowledge of the operating standards for the One Roof Program and the program’s expected client outcomes.
- Knowledge of resources in the community for youth and families who are experiencing complex needs, skilled at brokering services for client appropriate to the responsibilities of the job.
