Indigenous Chronic Kidney Disease Care Lead

Kingston Health Sciences Centre (KHSC) invites you to join our dedicated team as an Indigenous Chronic Kidney Disease (CKD) Care Lead.**
**Role Overview**:
As the Indigenous CKD Care Lead, you'll be pivotal in integrating care for individuals with chronic kidney disease across diverse healthcare systems. Your role will bridge gaps between Indigenous communities and healthcare services, ensuring culturally safe and supportive care. By enhancing early CKD identification and improving access to quality renal care, you’ll help create a seamless healthcare experience for First Nations, Inuit, Métis, and Urban Indigenous communities.
**Key Responsibilities**:
**Strategic Leadership**: Collaborate with internal and external partners to develop and implement quality improvement initiatives in CKD care. This includes risk-screening, chronic disease management, home dialysis, transplant, and palliative care.
**Project Management**: Lead and coordinate projects, ensuring adherence to objectives, effective resource use, and compliance with organizational policies. Manage project scope changes, budget revisions, and track progress.
Community Engagement: Work closely with Indigenous communities and health care providers across Southeast and Northern Ontario. Promote community-based renal care initiatives and ensure culturally appropriate practices are integrated into care delivery.
**Cultural Safety**: Promote Indigenous cultural safety within the Regional Renal Program. Support the delivery and evaluation of renal patient care, including policy development and best practice promotion.
**Advocacy and Support**: Provide advocacy and support for Indigenous patients, caregivers, and their families. Collaborate with local and provincial organizations to advance Indigenous kidney health care and report on quality indicators.
**BASIC QUALIFICATIONS**:
**Experience**:3-5 years in Indigenous health or social services, hospital or community-based case management, health promotion, public health, or social work. Indigenous lived experience required; facilitation experience and ability to speak an Indigenous language preferred.
**Skills**:Demonstrated problem-solving, critical thinking, and time management. Proficiency in report writing, budget management, and use of electronic health records. Strong communication, partnership building, and inter-professional care skills.
**Knowledge**:Familiarity with Indigenous cultural practices, system navigation, community resources, and Trauma Informed Care. Understanding of Indigenous health legislation and funding arrangements is essential.
