Job Summary

Within the Department of Family Medicine, the Queen’s Family Health Team (QFHT) is a multidisciplinary team of health care professionals who provide primary health care services to the Belleville community. As an academic teaching centre, the Department trains future family physicians and conducts research in issues of primary health care delivery. QFHT is comprised of a number of medical practices and provides for a diverse, client centred work environment for all its employees. QFHT provides primary health services to rostered patients while modeling safe, efficient and collaborative clinical processes to Family Medicine residents and other health professional learners.

Reporting to the Program Manager, the incumbent will practice patient-centred care to achieve optimal mental health service to enrolled patients, as part of a team of primary care health professionals. The incumbent will provide mental health assessment and counseling to patients, both individual and group, and is a resource to the clinical team.

In partnership with patients and other health care providers, the Mental Health Worker uses unique knowledge and skills to assist and resolve social issues that are impacting health, to provide counseling services and to advocate for patients and their families.

**Job Description**:

**KEY RESPONSIBILITIES**:

- In consultation with the interdisciplinary team (ie., physicians, residents, nurses), provides assessment and short-term counseling to improve patients’ mental health concerns such as anxiety, depression and situational distress.
- Provides assistance in resolving social issues that impact on health, such as form completion, accessing food/income security, liaison with Children’s Aid.
- Acts as an advocate for patients and their families in matters affecting health.
- Actively participates in planning, implementation, delivery and evaluation of mental health programs and services.
- Communicates with hospital and community mental health workers and other interdisciplinary team members to ensure appropriate access to community resources.
- Promotes health and wellness through education and teaching sessions.
- Provides patient education through individual and/or group sessions.
- Records and documents care provided within the patients’ electronic medical record.
- Maintains confidentiality in all aspects of patient, staff and organization information.
- Collaborates with physicians, residents and allied health professionals (ie., dietitian, pharmacist, nursing).
- Maintains and develops professional competence through ongoing professional development. Participates in self-directed learning, interdisciplinary committees and clinical audits.

**REQUIRED QUALIFICATIONS**:

- Regulated Health Professional (i.e. Social Worker, Registered Nurse, Occupational Therapist, etc.) with training from a recognized University.
- Registered and in good standing with the appropriate College.
- Minimum of 5 years experience working with patients and families in a health setting, preferably in a community and/or primary care setting. Experience with mental health patients preferred.
- Demonstrated competence in conflict management, interpersonal effectiveness and group therapy/education.
- Additional training or credentialing in cognitive behavioural training considered an asset.

**SPECIAL SKILLS**:

- Knowledge and proficiency in current, evidenced-based methods and practices of primary care social work services including clinical and supportive care.
- Excellent verbal and written communication skills; outstanding interpersonal skills.
- Ability to work both independently and effectively in an interdisciplinary team environment.
- In depth knowledge of the local health service delivery and community based services.
- Ability to be flexible and creative in order to develop strategies to meet the needs of a diverse patient, family and provider population.
- Proficient in the use of computer/technology (Word, Outlook, Internet) including prior experience and knowledge of electronic medical records.
- Excellent organization, time management and priority setting skills.
- Willingness to update knowledge and skills through various courses, workshops and conferences.

**DECISION MAKING**:

- Makes decisions regularly regarding patient care within the legal scope of practice of the profession.
