Job Summary

Pathways to Independence is seeking a highly skilled and compassionate Support Worker to join our team. As a Support Worker, you will play a vital role in providing assisted living support to adults with dual diagnosis, mental health, and behavioral challenges.

Key Responsibilities
  • Provide Support and Care: Assist individuals with daily living activities, such as homemaking, personal care, and community integration.
  • Implement Behavior Management Plans: Work with clients to develop and implement behavior management plans to promote positive behavior and independence.
  • Document Client Information: Maintain accurate and up-to-date client records, including progress notes and service plans.
  • Collaborate with Team Members: Work collaboratively with other Support Workers, Client Services Managers, and Counselors to ensure seamless service delivery and client support.
  • Advocate for Clients: Act as an advocate for clients, promoting their rights, choices, and self-determination.
Requirements
  • Education: Secondary School Graduation and a minimum one-year post-secondary certificate in Human or Health Sciences (Personal Support Worker or equivalent).
  • Current Certifications: Current First Aid and CPR certifications.
  • Valid Driver's License: Valid G class driver's license.
  • COVID Vaccination: Proof of full COVID vaccination.
Preferred Qualifications
  • Experience: Experience working with adults with dual diagnosis, mental health, or other cognitive impairment in a residential, community, or independent living environment.
  • Skills: Excellent written and oral communication skills, ability to work independently and within a team environment, and proficiency in electronic mail and other computer-based tools.
