The Pastoral Care Coordinator is accountable to the Director of Programs & Support Services for coordinating a variety of services, which allow the Long Term Care residents to meet their spiritual and emotional needs. Background/Qualifications: Education relevant to spiritual and religious growth. Endorsement by the coordinator’s faith group. Training and/or experience in gerontology. Training and/or experience in multi-faith programming. Key Responsibilities: The role of the Pastoral Care Coordinator shall include but not be limited to:

- Welcoming residents upon admission to the home, explaining to them the available pastoral services; identifying the resident’s faith group and contact.
- Facilitating contacts with the resident’s former faith group.
- Providing support to staff members in spiritual and religious care issues.
- Providing grief and bereavement support to staff, residents and families of residents when necessary and providing crisis intervention in experiences such as grief, illness and death.
- Providing input for dealing with ethical issues for residents, staff and families.
- Coordinating and directing faith groups’ programs of support to residents, including services, visitation, rites and sacraments.
- If the coordinator is not qualified, accessing the resources to provide the above services for the residents.
- Participating in the team planning to communicate the resident’s wishes and special needs; contributing notes in the resident’s record when appropriate.
- Providing memorial services at regular intervals.
- Planning, implementing and evaluating a variety of spiritual programs, making necessary adaptations to meet the resident’s needs.
- Participating in Palliative Care Committee.
- Participating in a Spiritual and Religious Care Advisory Committee.
- Assists residents during mealtimes. This includes, but is not limited to, set-up, prompting, assisting with feeding the resident, portering and meal supervision.
- Porter residents throughout the home, as needed.
- Assist with resident lifts, transfers and bed entrapment
- Perform other duties as assigned

Must adhere to the policies and procedures regarding infection, prevention and control within the home including wearing appropriate Personal, Protective Equipment as recommended by the home.

**Salary**: From $18.53 per hour
