The Alzheimer Society of Brant, Haldimand Norfolk, Hamilton Halton (ASBHNHH) is looking for a Dementia Resource Consultant. Under the direction of the Team Lead, First Contact, this position serves as the initial point of contact for hospital staff and potential clients of the ASBHNHH (persons living with dementia and/or care partners) in the Emergency Department (ED) of Brantford General Hospital (BGH) and Norfolk General Hospital (NGH). The Dementia Resource Consultant works in the Emergency Departments of the Brantford General Hospital and Norfolk General Hospital as part of the Integrated Dementia Resource team (DREAM Team).

The Dementia Resource Consultant will provide support connecting persons living with dementia and/or care partners to program and services within the Alzheimer society (such as, Behavioural Support Outreach, Counselling, Respite) and other community resources. The Dementia Resource Consultant follows their journey through service coordination and collaboration with other staff of ASBHNHH and community partner organizations. The Dementia Resource Consultant also educates/models for the Emergency Department team members on behavioural strategies to mitigate the risk or necessity for restraints in the Emergency Department through the use of Montessori activities and GPA strategies. The Dementia Resource Consultant may also be involved with existing Memory and Geriatric clinics in the community to administer cognitive testing as needed.

The Dementia Resource Consultant position requires flexibility to work out numerous locations in Brantf and Norfolk counties. The successful applicant is required to have a valid driver's license and access to a vehicle. Regular office hours are Monday to Friday from 8:30 am to 4:30 pm, but occasional evening and weekend work may be required. The hourly range for this role is $23.74-$35.60 ($43,200 to $64,800 based on 1820 hours annually). We also offer a competitive Group Benefits and RRSP package.

**Specialized Knowledge and Skill Requirements**:

- Demonstrated and extensive knowledge and skill working with elderly individuals with various types of dementia
- Demonstrated and extensive knowledge and skills in case management techniques including the identification, consultation, and assessment of the high-risk elderly
- Demonstrated experience working with persons living with dementia is required along with an in-depth knowledge of Alzheimer’s disease and related dementias including behavioural strategies and GPA techniques.
- Demonstrated and extensive knowledge of Community Support Services, Primary Health Care and Mental Health systems
- Knowledge of consent and capacity issues and legislation related to high-risk elderly individuals
- Excellent verbal and written communication skills
- Ability to work in a fast pace, multi-disciplinary team
- Ability to work independently in a professional, self-directed yet collaborative manner
- Ability to be flexible, creative and practical
- Valid Ontario Drivers’ license and access to a car
- Clear criminal reference check for the vulnerable sector
- Knowledge of PIECES framework and templates is a strong asset
- Familiarity with standardized assessment tools (e.g. Folstein, Cornell, Cohen-Mansfield, MoCA)

**Educational Requirements**:

- A university degree in social sciences or a related field, or comparable education, along with knowledge and understanding of community resources and services is required.
- Training in Gentle Persuasive Approaches (GPA) is preferred.
