**Job Summary**:
The Aging in Place (AIP) Program works to provide enhanced access to a broad range of community services for frail/at risk seniors living in Ottawa Community Housing. The Community Support Outreach Coordinator will share the vision of Ottawa West Community Support and support its mission. As a staff member of OWCS, the Community Support Outreach Coordinator will work to provide enhanced access to a broad range of community services for frail/at risk seniors living in Ottawa Community Housing. He/she will work to enhance the dignity of clients and to foster a safe, supportive and positive environment for them.

**Community Support Outreach Coordinator will**:

- Work with an open, friendly approach toward clients/tenants and volunteers, contract workers, Community Partners as they participate in the programs and services offered in the building(s)
- Disseminate relevant information to the tenants and the community
- Meet regularly with volunteers, tenants, tenant associations, partners, etc to implement programs and group activities in response to identified support needs
- Promote CSS programs and services to potential community referring sources, funders and other network members
- Collect and maintain program records, client files and statistical data

**Qualifications**

**Education**:
Degree/Diploma in one of the following areas: Social Services, Social Work, Community Development, Gerontology or Nursing.

**Experience**:

- 1-2 years working with older adults or persons with a physical disability.
- Employee supervision, an asset

**Operational Requirements**:

- Clear Criminal Record Check
- C.P.R and First Aid Certificate
- Bilingual (English and French) is a requirement for one position
- Bilingual (English and Mandarin) is a requirement for one position
- Additional languages spoken is an asset
- Access to a vehicle is a requirement

**Skills Required**:

- Program planning and facilitation
- Client assessment
- Excellent communication skills in both official languages
- Computer literacy

**Abilities**:

- To understand the needs, interests and concerns of older adults and persons with a physical disability
- To work in high demand and multi-task situations
- To remain composed in high stress situations
- To assess situations and make decisions
- To coordinate workload and meet deadlines
