**Position Overview**:
The Program Specialist functions as a member of a team, reporting to the leadership of the Primary Health Care branch, Manitoba Health. The Program Specialist is responsible for leadership, consultation and coordination of activities related to primary care and primary care home policy. They will play a lead role in the development and expansion of primary care initiatives and provide consultation to primary care providers in business practice change management. The Program Specialist is also responsible for mid
- to long-range strategic planning and policy development in the area of primary care and chronic disease. The Program Specialist liaises and develops effective and collaborative working relationships with local, regional, provincial, national and international stakeholders on primary care issues in order to enable better integration of services. Some travel within the province and Canada may be required.

The incumbent is accountable for the following key functions and responsibilities:

- Project management (includes developing project plan, monitoring implementation, problem solving and reporting, facilitating meetings)
- Development, implementation and evaluation of provincial policies, standards, protocols and frameworks
- Research (including needs assessment, literature review, critical review and analysis, developing options)
- Writing, reporting and communications (includes advisory notes, TB submissions, issue sheets)
- Developing consultative relationships

**Experience**:

- 4 years of experience in a health related setting
- Experience in and knowledge of service coordination, specifically for hard-to-reach and underserved populations.
- Experience in and knowledge of the development, implementation and evaluation of policies, standards, protocols and/or frameworks.

**Education (Degree/Diploma/Certificate)**:

- Bachelor’s degree in a health care related social science or administration field is required.
- Master’s degree in a relevant health care field preferred.
- An equivalent combination of education, experience and training may be considered.
