**Position Summary**:
Reporting to the Director of Planning, the Project Manager, Population Health provides comprehensive day-to-day strategic and operational support to the NOHT-ÉSON’s Planning Table and various working groups. The Planning Table and working groups are comprised of a broad range of health and social service providers, and patient/family/client/caregiver representatives.

NOTE: This is primarily a remote position, however, regular in person attendance MAY BE REQUIRED throughout the contract. This may include facilitating in person workshops, and attending education sessions/meetings.

**Duties and Responsibilities**
- Facilitate population health management and continuous quality improvement activities with various NOHT-ÉSON working groups (e.g. population segmentation, identifying and defining opportunities for improvement, collecting and analyzing data, determining the root cause of an issue, implementing solutions).
- Conduct literature reviews, environmental scans, and jurisdictional reviews, and synthesize information.
- Support the NOHT-ÉSON in developing and implementing change concepts, create and implement work plans.
- Develop evaluation frameworks and indicators, track and monitor performance, course correct when required.
- and regional-level performance measurement indicators.
- Ensure inclusion of lived experience, that Indigenous and Francophone voices are identified and expressed in all work.
- Identify novel datasets that can be leveraged to better inform population health and value-based care initiatives.
- Engage other areas of the NOHT-ÉSON in order to advance Population Health and Value Based Health Systems data-related priorities.
- Build relationships with interested and affected parties, including Ontario Health Teams, Ontario Health Regions, Ontario Health, and the Ministry of Health.
- Develop and manages broader partner relationships, including clinicians and administrators at various levels, in order to design, pilot and execute various initiatives.
- Write various reports and deliver presentations to targeted audiences.
- Write funding proposals.
- Other duties as assigned.

**Education and Training**
- Master’s degree in Healthcare or Business Administration, Public Health, Epidemiology or another related field, or Bachelor’s degree with relevant experience.
- Project Management Certification would be an asset.
- Lean Six Sigma Certification would be an asset.
- Change Management Certification would be an asset.
- Community engagement training would be an asset, e.g. IAP2.

**Knowledge and Experience**
- Minimum five (5) years of Project or Change Management Experience; Experience in a health care setting is an asset.
- Expertise in Population Health Management, Public Health, Community Health, health promotion, health planning, etc.
- Experience using project management methodologies and tools.
- Experience in data collection and analysis, and in using health equity frameworks.
- Experience using continuous quality improvement methodologies and tools.
- Experience in engagement of community, interested, and affected parties.
- Experience in health care evaluation and reporting, particularly the Quintuple Aim.
