**Case Manager overall responsibilities**:
The Case Manager has an assigned caseload of individuals with intellectual disabilities, and is responsible for carrying out career exploration efforts with the individual, for community-based employment opportunities. The service focuses on providing employment training and supports to facilitate the placement of the individual, and proactively maintain follow up supports for the individual, and communicate with the employers regularly to maintain client job placement retention.
**Basic Duties**:
- Works closely with the individual to gain an understanding of needs, skills and interests; carries out person centered assessment and planning as it relates to evaluating potential employment opportunities, including but not limited to unpaid work experience.
- Conduct career exploration with the individual through the use of various tools, and assists the individual with identifying interests and goals.
- Provides on-site training which enables the client to learn job skills and employer expectations, while integrating naturally into the work site environment.
- Provide advocacy and other support services when necessary to address clients individual service needs.
- Provide regular contact and communication to the clients support network including family/caregivers, family service worker, residential care providers and employers.
- Regularly communicate and submit monthly status reports, regarding the progress and needs of the clients, or any incidents to the Program Manager & Coordinator.
- Breaks the job/tasks into teachable parts, to be learned and performed by client(s).
- Identify learning barriers and determine suitable training method for the client(s).
- Implement training plans to obtain desired work performance to accommodate the needs and abilities of each client(s).
- Educates natural supports about the communications and support needs of the client(s).
- Secures feedback from the employer and co-workers regarding the performance of the client(s).
- Develops plans when appropriate, to implement gradual fading.
- Provide transportation training to assist client(s) in getting to and from the work site.
- Encourage client independence at the work site, and address work place issues, as required.
- Provides ongoing follow up and contact with the individual and employer and others in order to facilitate job retention, providing advocacy and other support services
**Qualifications for successful applicants**:
- High School Diploma, University or College in the Social Services field is an asset.
- Experience working with adults with an intellectual disabilities.
- Clear Criminal Record Check, Child and Adult Abuse Registry Checks, current CPR and First Aid certification is required.
- Vehicle and Valid Drivers License (Class 5) required - mileage reimbursed by employer.
